梯形体位垫在腹腔镜辅助胃癌手术中的应用

目的:探讨自制梯形体位垫优化“人”字形体位在腹腔镜辅助胃癌手术中应用效果。方法:对医院72例接受腹腔镜辅助胃癌根治手术患者进行统计分析,并随机分为观察组和对照组,所有患者均由同一组手术医师实施手术,观察组对“人”字形分腿体位进行改良,采用自制梯形体位垫放置于手术床腿板上,观察气腹建立前后对血流动力学的影响,并根据术后回访对比相关并发症的发生率。结果:观察组患者比对照组患者气腹建立前后对血流动力学波动较小,差异有统计学意义(P<0.05)。另外,观察组患者术后体位并发症的发生率低于对照组,差异有统计学意义(P<0.05)。结论:自制梯形体位垫在“人”字形分腿体位手术中可以确保患者肢体功能位,显著减少气腹建立前后对血流动力学的改变,提高患者术后舒适度。

Objective:To investigate the effect of self-made trapezoidal body pad on optimizing the"human"shape in laparoscopic-assisted surgery for gastric cancer.Methods:Statistically analyzed 72 patients who underwent laparoscopic assisted in patients with gastric cancer radical surgery In the first affiliated hospital of university of science and technology of china,they were randomly divided into observation group and control group,all patients were made by the same set of surgery doctors surgery,observation group on"people"glyph cent leg position improved,using self-made trapezoid posture mat is placed on the surgery bed legs plate,The influence of pneumoperitoneum on hemodynamics before and after establishment was observed,and the incidence of related complications was compared according to postoperative return visit.Results:The hemodynamic fluctuation before and after pneumoperitoneum was smaller in the observation group than in the control group,and it was statistically significant(P<0.05).In addition,the incidence of postoperative postural complications in the observation group was low and statistically significantthe(P<0.05).Conclusion:The self-made trapezoid position pad can ensure the functional position of the limbs,significantly reduce the hemodynamic changes before and after the establishment of pneumoperitoneum,and improve the postoperative comfort of the patients.
