scvo2、CRP 与SAA 联合检测在脓毒血症预后评估中的应用分析

Analysis of the application of SCVO2,CRP and SAA in the prognosis assessment of sepsis

目的:分析中心静脉血氧饱和度(scvo2)、C反应蛋白(CRP)与血清淀粉样蛋白A(SAA)联合检测在脓毒血症预后评估中的应用价值。方法:选取2018年5月至2020年5月期间本院接收的76例脓毒血症患儿作为研究对象,对其临床资料进行回顾性分析,根据其严重程度分为严重脓毒症组(44例)和一般脓毒症组(32例),并比较两组在入院时、治疗后6d内的scvo2、CRP与SAA水平,评估三个指标联合检测对患儿预后预测价值。结果:在入院时严重脓毒症组的scvo2水平低于一般脓毒症组,其CRP与SAA水平明显高于一般脓毒症组(P<0.05)。治疗3d内、治疗4d,一般脓毒症组的ScvO2水平逐渐上升,CRP、SAA逐渐降低,且维持在正常水平;严重脓毒症组的ScvO2水平3d内无明显变化,第4d逐渐上升,其CRP、SAA早期呈上升趋势,第4d缓慢下降(P<0.05)。结论:在脓毒血症预后评估中,联合检测scvo2、CRP与SAA,能根据患者的指标水平变化情况准确了解其病情,并做出正确的预后评估。

Objective:To analyze the application value of combined detection of venous oxygen saturation(SCVO2),C-reactive protein(CRP)and serum amyloid A(SAA)in the prognosis assessment of sepsis.Methods:In May 2018-May 2020,76 cases sepsis were selected in our hospital to receive as the research object,the clinical data were retrospectively analyzed,according to its severity in severe sepsis groups(44 cases)and general sepsis group(32 cases),and compare the two groups after admission,scvo2 within 6 days after treatment,CRP and SAA levels,evaluation of three indicators combined detection of children with prognostic value.Results:The SCVO2 level in the severe sepsis group at admission was lower than that in the general sepsis group,and the CRP and SAA levels were significantly higher than that in the general sepsis group(P<0.05).Within 3d and 4d of treatment,ScvO2 level in general sepsis group increased gradually,while CRP and SAA decreased gradually and remained at the normal level.In the severe sepsis group,ScvO2 level did not change significantly within 3d,but gradually increased on the 4th day,while CRP and SAA showed an early trend of increase and slowly decreased on the 4th day(P<0.05).Conclusion:In the prognosis assessment of sepsis,the combined detection of SCVO2,CRP and SAA can accurately understand the patients’condition according to the changes in the indicator level and make a correct prognosis assessment.
