新冠肺炎危重症治疗的探讨

目的探讨新冠肺炎危重症患者的治疗方法及效果。方法选择随州中心医院新冠ICU 2020年1月—3月进行治疗的50例新冠肺炎危重症患者纳入本次研究,所有患者入院后均进行常规抗病毒和对症支持治疗,并在此基础上使用血必净注射液。观察患者疗程结束时治疗效果、核酸检测转阴率以及治疗前后凝血功能指标、D-二聚体(D-D)、炎症指标检测结果变化情况。结果本组50例患者疗程结束时治疗有效率为68.00%、核酸检测转阴率为76.00%;治疗后患者凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、D-D检测结果均低于治疗前,差异显著(P<0.05);治疗后患者白细胞计数(WBC)检测结果高于治疗前,肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、红细胞沉降率(ESR)均低于治疗前,差异显著(P<0.05)。结论在新冠肺炎危重症治疗中使用血必净注射液疗效显著,能够有效改善凝血功能和炎症状态,值得临床推荐。

Objective To explore the novel coronavirus pneumonia treatment.Methods 50 novel coronavirus pneumonia patients with severe acute respiratory syndrome who were treated in the new crown of Suizhou Central Hospital in January ICU2020 in January were enrolled in the study.The study included the following:the first study was conducted in March.All patients were treated with routine antiviral and symptomatic support after admission,and on this basis,Xuebijing injection was used.The treatment effect,the negative rate of nucleic acid detection and the changes of coagulation function index,D-Dimer(D-D)and inflammation index before and after treatment were observed.Results At the end of treatment,the effective rate was 68.00%,and the negative rate of nucleic acid detection was 76.00%;Prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT)after treatment、The results of FIB and D-D were lower than those before treatment,The difference was significant(P<0.05);After treatment,the white blood cell count(WBC)test results were higher than before treatment,tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)detection results were lower than before treatment,The difference was significant(P<0.05).Conclusion novel coronavirus injection is effective in novel coronavirus pneumonia treatment,and it can effectively improve coagulation function and inflammation state of patients.
