超声引导下区域神经阻滞麻醉用于单侧膝关节及其以下手术的临床效果

Clinical effect of ultrasound-guided regional nerve block anesthesia for unilateral knee joint and below surgery

目的分析在单侧膝关节及其以下手术患者中应用超声引导下区域神经阻滞麻醉的临床效果。方法将2019年3月—2020年3月期间在寻乌县人民医院行单侧膝关节及其以下手术的40例患者随机设为2个组别,给予对照组(20例)腰硬联合麻醉,给予试验组(20例)超声引导下区域神经阻滞麻。观察2组患者麻醉操作时间、感觉神经阻滞、运动神经阻滞情况,评价麻醉效果和不良反应。结果试验组麻醉操作时间高于对照组,具有统计学差异(P<0.05)。试验组感觉神经阻滞起效时间短于对照组,维持时间长于对照组,运动神经阻滞起效时间和维持时间短于对照组,均有统计学差异(P<0.05)。试验组不良反应总发生率低于对照组,具有统计学差异(P<0.05)。对比2组患者麻醉优良率,无统计学差异(P>0.05)。结论超声引导下区域神经阻滞麻醉应用于单侧膝关节及其以下手术患者中,具有良好的麻醉效果,并且不良反应较少,值得推广。

Objective To analyze the clinical effects of ultrasound-guided regional nerve block anesthesia in patients with unilateral knee joint and below surgery.Methods From March 2019 to March 2020,40 patients who underwent unilateral knee surgery and below in our hospital were randomly divided into two groups.The control group(20 patients)was given combined spinal-epidural anesthesia and the experiment was given Group(20 cases)was anesthesia with regional nerve block under ultrasound guidance.Observe the anesthesia operation time,sensory nerve block,motor nerve block of the two groups of patients,and evaluate the anesthesia effect and adverse reactions.Results The operation time of anesthesia in the experimental group was higher than that in the control group,and there was a statistical difference between the two groups(P<0.05).The onset time and maintenance time of sensory nerve block in the experimental group were higher than those in the control group,and the onset time and maintenance time of motor nerve block were lower than those in the control group.There was a statistical difference between the two groups(P<0.05).The total incidence of adverse reactions in the experimental group was lower than that in the control group,and there was a statistical difference between the two groups(P<0.05).There was no statistical difference in the excellent and good rate of anesthesia between the two groups(P>0.05).Conclusion Ultrasound-guided regional nerve block anesthesia is used in patients with unilateral knee joint surgery and below.It has a good anesthetic effect and has fewer adverse reactions.It is worthy of promotion.
