多层螺旋CT增强扫描应用于胃癌淋巴结转移诊断的可行性研究

Study on Feasibility of Enhanced Multi-slice Spiral CT Scan in the Diagnosis of Lymph Node Metastasis in Gastric Cancer

目的:探究多层螺旋CT增强扫描应用于胃癌淋巴结转移诊断的可行性。方法:回顾性方法选择并分析2018年9月-2019年9月来我院就诊的60例胃癌病人,采用病理检测方法检测患者的胃癌淋巴结是否转移,再采用多层螺旋CT增强扫描检测患者的对患者淋巴结转移的发生率与淋巴结转移患者的对比剂平均通过时间(Mean Transit Time,MTT)、血容量(blood volume,BV)、表面通透性(permeability surface,PS)、出血流量(blood flow,BF),并应用ROC曲线,分析多层螺旋CT增强扫描对胃癌淋巴结转移的诊断价值。结果:有淋巴结转移的患者MTT、PS水平大于无淋巴结转移患者,BF水平小于无淋巴结转移患者,差异有统计学意义(P<0.05);ROC曲线分析显示,MTT、PS、BF对胃癌淋巴结转移的诊断价值的AUC分别为0.753、0.696、0.753,三项联合对胃癌淋巴结转移的诊断价值的AUC为0.789,对胃癌淋巴结转移的诊断价值准确性高。结论:多层螺旋CT增强扫描对胃癌淋巴结转移诊断有较高的诊断价值。

Objective:To explore the feasibility of enhancedmulti-slice spiral CT scan in the diagnosis of lymph node metastasis in gastric cancer.Methods:A retrospective method was used to select and analyze 60 patients with gastric cancer who came to our hospital from September 2018 to September 2019.Pathological detection method was used to detect whetherlymph node metastasis occurred in patients with gastric cancer,and enhancedmulti-slice spiral CT scan was used to detect the incidence rate of lymph node metastasis in patients and the mean transit time of contrast agent(MTT),blood volume(BV),permeability surface(PS)and blood flow(BF).ROC curve was applied to analyze the diagnostic value of enhanced multi-slice spiral CT scan on lymph node metastasis in gastric cancer.Results:The MTT and PS of patients with lymph node metastasis were higher than those of patients without lymph node metastasis while the BF was lower than that of patients without lymph node metastasis(P<0.05).ROC curve analysis showed that the AUC values of diagnostic value of MTT,PS and BF on lymph node metastasis in gastric cancer were 0.753,0.696 and 0.753 respectively.The AUC of the combination of the three itemswas 0.789 in the diagnosis of lymph node metastasis in gastric cancer,andthe combination of the three had high diagnostic accuracyon lymph node metastasis in gastric cancer.Conclusion:Enhanced multi-slice spiral CT scan has a high diagnostic value in the diagnosis of lymph node metastasis in gastric cancer.
