摘要
目的分析电子计算机断层扫描(CT)和磁共振成像(MRI)对肝癌动脉化疗栓塞术(TACE)后治疗疗效的评估价值方法选取2013年1月至2017年12月我院肝病科127例采用TACE3-5术后的肝癌患者作为研究对象,患者治疗后行CT和MRI影像学检查以及数字减影血管造影检查(DSA),比较分析CT和MRI检查的病灶残余、复发情况以及患者术后肿瘤病灶包膜检出情况,比较分析不同碘油沉积类型患者的病灶残余、复发情况。结果MRI诊断的准确率、敏感性明显高于CT扫描(P<0.05);MRI以及CT特异性均为100%。本研究中127例患者存在157个病灶,MRI对患者术后肿瘤包膜检出情况明显高于CT检查(P<0.05);MRI对患者术后不同碘油沉积类型检出情况明显高于CT检查,差异具有统计学意义(P<0.05)。结论MRI影像学检查评估TACE术后病灶残余以及复发情况具有较好准确率和敏感性,可有效显示肿瘤病灶包膜情况和不同类型碘油沉积的病灶残余与复发情况,MRI对肝癌TACE术后治疗疗效评估价值高于CT,可为患者预后判断提供有效依据。
Objective To evaluate the therapeutic value of computed tomography(CT)and magnetic resonance imaging(MRI)after Transhepatic Arterial Chemotherapy And Embolization(TACE).Methods A total of 127 patients with liver cancer between January 2013 and December 2017 who underwent 3-5 times TACE admitted in our liver disease department were enrolled to proceed with CT&MRI imaging and digital subtraction angiography(DSA).The lesion residual and recurrence of CT and MRI and postoperative lesion envelope were compared and analyzed to analyze the lesion residual and recurrence in patients with different types of lipiodol deposition were compa red and analyzed.Results The accu racy and sensitivity of MRI diagnosis were significantly higher than CT scan(P<0.05);MRI and CT specificity were 100%.There were 157 lesions in 127 patients in this study,and the detection of tumor envelope by MRI was significantly higher than that of CT examination(P<0.05).MRI Detection was considerably higher than that of CT examinations for different postoperative lipiodol deposition types(P<0.05).Conclusion MRI imaging has good accuracy and sensitivity in evaluating the residual and recurrence of TACE,which can effectively show the tumor envelope and the residual and recurrence of different types of lipiodol deposition.The thera peutic efficacy evaluation of MRI on TACE after liver cancer is higher than CT,which can provide a sufficient basis for patient prognosis.
