球拍形切口乳房成形术治疗早期乳腺癌的效果及对患者乳头横向偏移距离的影响

Effect of Racket-shaped Incision Mammoplasty in the Treatment of Early Breast Cancer and Its Influence on the Distance of Patients'Nipple Lateral Deviation

目的研究球拍形切口乳房成形术治疗早期乳腺癌的临床效果及对患者乳头横向偏移距离的影响。方法选取早期乳腺癌患者90例,采用随机数字表法将其分为对照组和实验组,各45例。对照组采用常规保乳手术治疗,实验组采用球拍形切口乳房成形术治疗,比较2组患者的手术时间、术中总出血量;对比2组患者的临床疗效;比较2组患者的美容效果及术后并发症发生情况。结果①2组患者的手术时间、术中总出血量差异无统计学意义(P>0.05)。②2组的复发率与转移率差异无统计学意义(P>0.05)。③术后2个月实验组患者乳头横向偏移距离评分、乳头纵向偏移距离评分、对称性评分、凹陷程度评分、瘢痕评分均高于对照组(P<0.05)。④实验组术后并发症发生率低于对照组(P>0.05)。结论与常规保乳术相比,球拍形切口形成术治疗早期乳腺癌的手术用时、术中出血量及临床疗效相当,但可减少乳头横向偏移距离,减少并发症的发生。

Objective To study the clinical effect of racket-shaped incision mammoplasty in the treatment of early breast cancer and its influence on the lateral offset distance of the patient's nipple.Methods 90 patients with early breast cancer were selected,and they were divided into the control group and the experimental group according to random number table method,each with 45 cases.The control group was treated with conventional breast-conserving surgery,and the experimental group was treated with racket-shaped incision mammoplasty.The operation time and total intraoperative blood loss of the two groups were compared;the clinical curative effect of the 2 groups was compared;the cosmetic effect of the 2 groups was compared.Postoperative complications occurred.Results①There was no statistically significant difference in operation time and total intraoperative blood loss between the 2 groups(P>0.05).②There was no significant difference in the recurrence rate and metastasis rate between the 2 groups(P>0.05).③Two months after the operation,the scores of lateral nipple offset distance,longitudinal nipple offset distance,symmetry score,depression degree score,and scar score of the experimental group were higher than those of the control group(P<0.05).④The incidence of postoperative complications in the experimental group was lower than that of the control group(P>0.05).Conclusion Racket-shaped incision formation for early breast cancer is equivalent to conventional breast-conserving surgery in terms of operation time,intraoperative blood loss and clinical curative effect,but the former can reduce the distance of nipple lateral deviation and reduce the occurrence of complications.
