摘要 目的探讨超声乳腺影像报告数据系统(BI-RADS)分类在乳腺癌鉴别诊断中的价值。方法采用回顾性研究方法,选取1880例乳腺肿块患者2445个乳腺肿块,比较BI-RADS分类诊断结果与病理诊断结果的差异。结果2445个乳腺肿块中,病理诊断2165个乳腺肿块为良性,280个肿块为恶性。BI-RADS分类诊断结果与病理结果Kappa值为0.857,P<0.05;BI-RADS分类诊断乳腺癌的灵敏性、特异性、准确性、阳性预测值和阴性预测值分别为85.71%、98.61%、97.14%、88.89%和98.16%;年龄<40岁组BI-RADS分类诊断乳腺癌的特异性、准确性和阳性预测值分别为100.00%、98.06%和100.00%,高于年龄≥40岁组(P<0.05)。结论BI-RADS分类在乳腺癌鉴别诊断中有较好的应用价值,尤其是对年龄<40岁的患者的鉴别诊断价值高。 Objective To explore the value of ultrasonic breast image report data system(BI-RADS)classification in the differential diagnosis of breast cancer.Methods Using retrospective research methods,1880 patients 2445 breast masses were selected,the difference between BI-RADS classification and pathological results was compared.Results Of 2445 breast masses,2165 were pathologic diagnosis benign and 280 were malignant;The Kappa value of the BI-RADS classified diagnosis and pathological results was 0.857,P<0.05;The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of BI-RADS classification for breast cancer were 85.71%,98.61%,97.14%,88.89%and 98.16%;The specificity,accuracy and positive predictive value of BI-RADS classification for breast cancer in the age group<40 years old were 100.00%,98.06%and 100.00%,which were higher than those in the age group≥40 years old(P<0.05).Conclusion BI-RADS classification has a good application value in breast cancer identification,especially in the age group<40 years old,its differential diagnosis value is high.
