胃癌患者血清CEA、AFP、CA724、CA125的检测联合病理学检查的临床诊断效果

Clinical Diagnostic Effect of Serum CEA,AFP,CA724,CA125 combined with Pathological Examination in Patients with Gastric Cancer

目的探讨胃癌患者血清癌胚抗原(CEA)、甲胎蛋白(AFP)、糖类抗原724(CA724)、糖类抗原125(CA125)的检测联合病理学检查的临床诊断效果。方法收集胃癌患者69例为研究对象纳入胃癌组,选择同期良性病变组患者58例为良性病变组,另选同期行健康检查的健康受试者50例为对照组。对胃癌组、良性病变组患者和对照组受试者的CEA、AFP、CA724、CA125水平进行检测对比,并对各组患者进行组织病理学检查。对各组受试者的病理学检测结果、各项肿瘤标志物指标水平进行组间统计对比。并对胃癌组患者各项指标单独诊断以及肿瘤标志物、病理学检查联合诊断的诊断阳性率、灵敏度、特异度、准确度进行计算和组间对比。结果胃癌组患者的肿瘤标志物CEA、AFP、CA724、CA125水平均高于良性病变组且高于对照组(P<0.05)。胃癌患者联合检查的诊断阳性率高于CEA、AFP、CA724、CA125、病理学检查的单独检测结果(P<0.05)。CEA、AFP、CA724、CA125、病理学联合检测的灵敏度、特异度、准确度均高于单一检测的相应结果(P<0.05)。结论血清CEA、AFP、CA724、CA125的检测联合病理学检查对于胃癌患者有较高的临床诊断价值,可进一步提高诊断正确率,为胃癌的临床诊断提供有效的参考。

Objective To investigate the clinical diagnostic effect of combined detection of serum carcinoembryonic antigen(CEA),alpha fetoprotein(AFP),carbohydrate antigen 724(CA724),carbohydrate antigen 125(CA125)in patients with gastric cancer.Methods 69 patients with gastric cancer were collected as research objects and included in the gastric cancer group.58 patients in the benign lesion group were selected as the benign lesion group,and 50 healthy subjects who underwent health examinations during the same period were selected as the control group.The levels of CEA,AFP,CA724 and CA125 in gastric cancer group,benign lesion group and control group were detected and compared,and histopathological examination was performed on each group.The pathological examination results and tumor markers were compared among the groups.The positive rate,sensitivity,specificity and accuracy of single diagnosis and combined diagnosis of tumor markers and pathological examination in gastric cancer group were calculated and compared.Results The levels of CEA,AFP,CA724 and CA125 in gastric cancer group were higher than those in benign lesion group and the control group(P<0.05).The positive rate of combined examination was higher than that of CEA,AFP,CA724,CA125 and pathological examination alone(P<0.05).The sensitivity,specificity and accuracy of combined detection of CEA,AFP,CA724,CA125 and pathology were higher than those of single detection(P<0.05).Conclusion The detection of serum CEA,AFP,CA724,CA125 combined with pathological examination has high clinical diagnostic value for gastric cancer patients,which can further improve the diagnostic accuracy and provide an effective reference for the clinical diagnosis of gastric cancer.
