对行手术治疗的胫腓骨骨折患者实施超声引导下区域神经阻滞麻醉的临床效果

Clinical Value of Ultrasound-guided Regional Nerve Block Anesthesia in the Treatment of Tibiofibular Fracture

目的研讨对行手术治疗的胫腓骨骨折患者实施超声引导下区域神经阻滞麻醉的临床效果。方法选择胫腓骨骨折患者52例,按照治疗方案不同将其分为2个小组。比对组26例实施腰硬联合麻醉,试验组26例实施超声引导下区域神经阻滞麻醉,对比不同麻醉方案的基本麻醉情况、患者生命指标以及麻醉安全性方面的影响。结果试验组疼痛评分(1.22±0.20)分、麻醉应用时间(7.01±0.68)min、术后清醒时间(9.60±1.02)min均分别优于比对组的(5.20±0.65)分、(15.20±2.11)min、(12.50±1.95)min,差异有统计学意义(P<0.05)。麻醉后试验组心率低于比对组[(84.25±3.60)次/分vs.(92.51±6.05)次/分],试验组血压高于比对组[(113.25±5.36)mm Hg vs.(107.52±4.05)mm Hg](P<0.05)。试验组麻醉安全性优于比对组(7.69%vs.26.92%)(P<0.05)。结论对行手术治疗的胫腓骨骨折患者实施超声引导下区域神经阻滞麻醉的临床效果显著,安全性与可行性强,麻醉质量高。

Objective To investigate the clinical value of ultrasound-guided regional nerve block anesthesia in the surgical treatment of tibiofibular fractures.Methods Fifty-two patients with tibia and fibula fracture were selected and divided into 2 groups according to different treatment schemes.Comparison group(n=26):combined spinal and epidural anesthesia.In the experimental group,26 cases were treated with regional nerve block anesthesia under ultrasound guidance.The effects of different anesthesia schemes on basic anesthesia,patients'vital indicators and anesthesia safety were compared.Results The pain score of the test group(1.22±0.20)points,the application time of anesthesia(7.01±0.68)min,and the postoperative wake-up time(9.60±1.02)min were better than those of the control group(5.20±0.65)and(15.20±2.11)min,(12.50±1.95)min,the difference was statistically significant(P<0.05).After anesthesia,the heart rate of the test group was lower than that of the control group[(84.25±3.60)beats/min vs.(92.51±6.05)beats/min],and the blood pressure of the test group was higher than that of the control group[(113.25±5.36)mm Hg vs.(107.52±4.05)mm Hg](P<0.05).The safety of anesthesia in the experimental group was better than that in the comparison group(7.69%vs.26.92%)(P<0.05).Conclusion The clinical effect of ultrasound-guided regional nerve block anesthesia for patients with tibia and fibula fractures undergoing surgical treatment is remarkable,safe and feasible,and the anesthesia quality is high.
