脂联素、TNF-α、FFA联合检测对妊娠期糖尿病的评估价值

Evaluation Value of Adiponectin, TNF-α and FFA in Gestational Diabetes Mellitus

目的探究脂联素、TNF-α、FFA联合检测对妊娠期糖尿病的评估价值。方法选取我院2017年1月至2020年3月收治的确诊妊娠期糖尿病患者47例为妊娠期糖尿病组,同期选取我院健康孕妇47名为对照组,检测并对比两组脂联素、TNF-α、FFA水平及两组诊断效能。结果妊娠期糖尿病组脂联素显著低于健康对照组,TNF-α、FFA水平显著高于健康对照组(P<0.05);以口服糖耐量试验为金标准,脂联素诊断敏感度、特异度、准确度分别为59.57%、60.00%、59.77%;TNF-α诊断敏感度、特异度、准确度分别为55.32%、45.00%、50.57%;FFA诊断敏感度、特异度、准确度分别为53.19%、60.00%、56.32%;联合诊断敏感度、特异度、准确度分别为87.23%、87.50%、87.35%,联合诊断敏感度、特异度、准确度显著高于单项检查,差异具备统计学意义(P<0.05)。结论在妊娠糖尿病评估诊断中,脂联素、TNF-α、FFA联合具有较高的诊断效能。

Objective To explore the evaluation value of adiponectin,TNF-α,FFA combined detection for gestational diabetes.Methods Select 47 cases of diagnosed gestational diabetes patients admitted to our hospital from January 2017 to March 2020 as the gestational diabetes group,and select 47 healthy pregnant women in our hospital as the control group during the same period to detect and compare the two groups of adiponectin and TNF-α,FFA levels and the diagnostic efficacy of the two groups.Results Adiponectin in the gestational diabetes group was significantly lower than that in the healthy control group,and the levels of TNF-αand FFA were significantly higher than those in the healthy control group(P<0.05);taking oral glucose tolerance test as the gold standard,the diagnostic sensitivity and specificity of adiponectin,accuracy were 59.57%,60.00%,59.77%;TNF-αdiagnostic sensitivity,specificity,accuracy were 55.32%,45.00%,50.57%;FFA diagnostic sensitivity,specificity,accuracy were 53.19%,60.00%,56.32%;the sensitivity,specificity,and accuracy of combined diagnosis were 87.23%,87.50%,and 87.35%,respectively.The sensitivity,specificity,and accuracy of combined diagnosis were significantly higher than that of single inspection,and the difference was statistically significant(P<0.05).Conclusion In the evaluation and diagnosis of gestational diabetes,the combination of adiponectin,TNF-α,and FFA has a high diagnostic efficiency.
