微创切口在下颌骨骨折整形修复中的应用研究

Application of Minimally Invasive Incision in Plastic Repair of Mandibular Fracture

目的:探究微创切口手术治疗下颌骨骨折的美容效果、安全性及患者满意率。方法:回顾性分析笔者医院2015年1月-2019年5月收治的125例下颌骨骨折患者临床资料,所有患者均采取手术治疗,其中62例采取口外颌周切口入路颌间结扎手术治疗,将其作为对照组,63例患者采取微小切口进路坚固内固定手术治疗,将其作为观察组,比较两组患者手术时间及术中出血量,术后美容效果、并发症发生率及术后疼痛持续时间,术后活动时间、软组织肿胀消退时间、骨折愈合时间及出院时间;随访分析比较两组患者术后1年手术美容效果满意率。结果:观察组患者手术时间与对照组相当(P>0.05),术后出血量少于对照组(P<0.05);观察组患者手术美容效果有效率96.83%,高于对照组的81.70%(P<0.05);观察组患者术后并发症发生率0.00%,低于对照组的8.06%(P<0.05);观察组患者术后疼痛持续时间、术后活动时间、软组织肿胀消退时间、骨折愈合时间及出院时间均短于对照组(P<0.01);观察组患者1年后手术美容效果满意率98.41%,高于对照组的85.48%(P<0.05)。结论:微创切口手术治疗下颌骨骨折美容效果良好,创伤小,患者术后恢复快,且满意率高,可作为治疗下颌骨骨折的首选方法,但选择何种手术方法应综合考虑患者实际及临床情况。

Objective To explore the cosmetic effect,safety and satisfaction rate of minimally invasive incision surgery for mandibular fracture.Methods Retrospective analysis from January 2015 to May 2019 treated 125 cases of mandibular fractures in patients with clinical data,all patients were treated with surgery,of which 62 cases take outlets incision into the jaw jaw weeks between ligation treatment,as a control group,63 patients were taken small incision approach strong internal fixation surgery,as a group,compared two groups of patients with operative time and intraoperative blood loss,postoperative patients with cosmetic effect,postoperative complications and duration of postoperative pain,postoperative patients with activity time,soft tissue swelling subsided time,fracture healing time and discharge time.The follow-up analysis compared the satisfaction rate of cosmetic surgery in the two groups 1 year after operation.Results The operation time of patients in the observation group was the same as that in the control group(P>0.05),and the postoperative blood loss was less than that in the control group(P<0.05).The effective rate of cosmetic surgery in the observation group(96.83%)was higher than that in the control group(81.70%)(P<0.05).The postoperative complication rate(0.00%)in the observation group was lower than that in the control group(8.06%)P<0.05.The duration of postoperative pain,postoperative activity time,regression time of soft tissue swelling,fracture healing time and discharge time in the observation group were all shorter than those in the control group(P<0.01).The satisfaction rate of cosmetic surgery in the observation group 98.41%was higher than that in the control group 85.48%(P<0.05).Conclusion Minimally invasive incision surgery for the treatment of mandibular fracture has a good cosmetic effect,with little trauma,good postoperative recovery,rapid recovery,and a high satisfaction rate,which is worthy of promotion and application.
