影响面部痤疮患者治疗依从性的相关因素分析

Analysis of Related Factors Affecting Treatment Compliance of Patients with Facial Acne

目的:探讨美观满意度与社会心理因素对面部痤疮患者治疗依从性的影响。方法:纳入2017年1月-2019年1月在笔者科室门诊就诊的痤疮患者239例,通过门诊/电话随访患者遵循医嘱治疗情况,评估患者治疗依从性,以依从性评分中位数划分为依从性良好组和依从性较差组,采用问卷调查方式收集两组患者临床资料,对比治疗后面部美观满意度、社会支持度及社会心理因素等相关指标;分析影响面部痤疮患者治疗依从性的相关因素。结果:依从性较差组患者担忧药物不良反应、焦虑自评量表(Self-rating anxiety scale,SAS)、抑郁自评量表(Self-rating depression scale,SDS)评分方面高于依从性良好组,治疗信心、耐心程度、皮肤美观满意度、社会支持度、自我认同感评分方面低于依从性良好组,差异具有统计学意义(P<0.05)。多因素线性回归方程显示,对治疗缺乏信心(t=-7.596)、SDS评分越高(t=-2.404)、SAS评分越高(t=-3.361)、对治疗缺乏耐心(t=-3.798)、皮肤美观满意度低(t=2.476)是影响痤疮患者治疗依从性的独立相关因素(P<0.05)。结论:社会心理因素、个人对皮肤美观满意度、负性情绪在很大程度上能够影响痤疮患者治疗依从性,临床工作中可针对性地采取措施,给予此类患者更多的支持和关怀。

Objective To explore the effect of aesthetic satisfaction and social psychological factors on the treatment compliance of patients with facial acne.Methods 239 acne patients who were treated in the outpatient department of dermatology department of dermatology,the first affiliated hospital of air force military medical university from January 2017 to January 2019 were included.The patients were followed up by outpatient/telephone follow-up treatment according to the doctor's instructions to evaluate the patient's treatment compliance.The median compliance score was divided into good compliance group and poor compliance group.A questionnaire survey was used to collect the clinical data of the two groups of patients,and the related indexes such as aesthetic satisfaction,social support and social psychological factors after treatment were compared;Analyze the relevant factors that affect the treatment compliance of facial acne patients.Results The patients in the poor compliance group had higher scores of worrying about adverse drug reactions,SAS and SDS than those in the good compliance group,the scores of treatment confidence,patience,skin beauty satisfaction,social support and self-identity were lower than those of the good compliance group,the differences were statistically significant(P<0.05).Multivariate linear regression equation showed that lack of confidence in treatment(t=-7.596),higher SDS score(t=-2.404),higher SAS score(t=-3.361),lack of patience with treatment(t=-3.798)and low satisfaction with skin appearance(t=2.476)were independent related factors influencing the treatment compliance of acne patients(P<0.05).Conclusion Socio-psychological factors,personal satisfaction with skin aesthetics and negative emotions can affect the treatment compliance of acne patients to a large extent.In clinical work,targeted measures can be taken to give such patients more support and care.
