经皮肾镜取石术在治疗复杂性肾结石中的临床效果

Clinical application of PCNL in complex kidney stones

目的:总结治疗复杂性肾结石过程中应用经皮肾镜取石术(Percutaneous nephrolithotripsy,PCNL)的疗效。方法:选择于我院接受治疗的30例复杂性肾结石患者作为研究对象,按照治疗方式的不同将其分为研究组(PCNL)与对照组(接受开放性肾切开治疗)各15例。比较两组患者接受治疗后住院时间、术中出血量、术后尿液转清时间、结石取净率以及术后并发症情况。结果:研究组患者治疗后住院时间、术中出血量少于对照组患者,差异有统计学意义(P<0.05),术后尿液转清时间与对照组相比差异无统计学意义(P>0.05);研究组患者治疗后结石取净率显著高于对照组,差异有统计学意义(P<0.05);研究组患者治疗后术后并发症发生率小于对照组,差异有统计学意义(P<0.05)。结论:实施PCNL在治疗复杂性肾结石过程中,具有更高的碎石取净率,缩短手术时间,减少术中出血量,减少不良反应的发生,是一种安全、有效的治疗方式。

Objective:To observe and study the effect of air PCNL in the treatment of complex kidney stones.Methods:Thirty patients with complex kidney stones who were treated in our hospital were selected as the research objects.They were divided into a study group(receiving pneumatic ballistic combined with holmium laser treatment)and a control group(receiving open renal resection)according to different treatment methods,15 cases each.The two groups were compared to observe the length of hospital stay,intraoperative blood loss,postoperative urine clearance time,stone removal rate,and postoperative complications.Results:The length of hospital stay and intraoperative blood loss in the study group after treatment were significantly different from those in the control group(P<0.05),and the postoperative urine clearance time was not statistically different from that in the control group(P>0.05);The rate of stone removal after treatment in the study group was significantly higher than that in the control group(P<0.05);the incidence of postoperative complications after treatment in the study group was significantly different from that in the control group(P<0.05).Conclusion:The implementation of PCNL in the treatment of complex kidney stones has a higher lithotripsy removal rate,shortens the operation time,reduces the amount of intraoperative bleeding,and reduces the occurrence of adverse reactions.It is a safe and effective treatment.
