摘要
目的:探究血清性激素联合彩超对特发性中枢性性早熟的诊断价值。方法:选取2018年8月至2020年8月就诊的特发性中枢性性早熟(ICCP)女童55例为ICCP组,非中枢性性早熟(包括外周性性早熟以及部分性性早熟)女童48例为PPP组及体检的同龄正常女童50例作为对照组。对其进行血清性激素和盆腔彩超检查,比较三组血清性激素、卵巢及子宫等,并分析血清性激素联合彩超对特发性中枢性性早熟的诊断价值。结果:三组雌二醇(E2)、促黄体生成素(LH)、卵泡刺激素(FSH)及垂体催乳素(PRL)比较差异具有统计学意义(P<0.05);三组卵巢容积、子宫容积及卵泡最大直径比较差异具有统计学意义(P<0.05);二者联合检测ICCP的灵敏度、特异度、阳性预测值及阴性预测值均高于二者单一检测(P<0.05)。结论:ICCP组患儿E2、LH、FSH、PRL水平、卵巢容积、子宫容积及卵泡最大直径高于PPP组患儿及正常女童,血清性激素联合彩超检测ICCP准确率较高,可作为ICCP诊断和鉴别的重要参考依据。
Objective:To explore the diagnostic value of serum sex hormones combined with color Doppler ultrasound on idiopathic central precocious puberty.Methods:A total of 55 girls with idiopathic central precocious puberty(ICCP)who were examined from August 2018 to August 2020 were selected as the ICCP group.48 girls with non-central precocious puberty(including peripheral precocious puberty and partial precocious puberty)were included in the PPP group and 50 normal girls of the same age were included in the control group.Serum sex hormones and pelvic color Doppler ultrasound were performed to compare the differences in serum sex hormones,ovaries and uterus among the three groups,and the diagnostic value of serum sex hormones combined with color Doppler ultrasound for idiopathic central precocious puberty was analyzed.Results:There were statistically significant differences in estradiol(E2),luteinizing hormone(LH),follicle stimulating hormone(FSH)and pituitary prolactin(PRL)among the three groups(P<0.05).There were significant differences in ovarian volume,uterine volume and follicle maximum diameter among the three groups(P<0.05).The sensitivity,specificity,positive predictive value and negative predictive value of the combined detection of ICCP were higher than those of single detection of ICCP(P<0.05).Conclusion:The levels of E2,LH,FSH,PRL,ovarian volume,uterine volume and the maximum diameter of follicles in the ICCP group were significantly higher than those in the PPP group and normal girls.The accuracy of serum sex hormones combined with color Doppler ultrasound to detect ICCP is higher,and it can be used for ICCP diagnosis and an important reference basis for identification.
