CYFRA21-1在肺癌患者血清中的表达及自身抗体与临床病理特征的相关性

Expression of CYFRA21-1 in the Serum of Lung Cancer Patients and the Correlation between Autoantibodies and Clinicopathological Characteristics

目的探讨细胞角蛋白21-1(CYFRA21-1)在肺癌患者血清中的表达及自身抗体与临床病理特征的相关性。方法选取肺癌患者78例作为肺癌组,同期选择性别比例匹配和年龄相近的健康人78例作为健康组,检测2组血清CYFRA21-1、自身抗体-DLL4蛋白与Notch蛋白表达水平,并与肺癌患者的临床病理特征进行相关性分析。结果肺癌组的CYFRA21-1、DLL4与Notch表达阳性率分别为85.9%、83.3%和62.8%,都显著高于健康组的5.1%、6.4%和5.1%(P<0.05)。在肺癌组中,不同分化类型、临床分期、淋巴结转移的肺癌患者CYFRA21-1、DLL4、Notch表达阳性率对比差异都有统计学意义(P<0.05)。Pearson相关性分析显示CYFRA21-1、DLL4、Notch表达阳性率与分化类型、临床分期、淋巴结转移都呈现相关性(P<0.05);Logistic回归分析显示分化类型、临床分期、淋巴结转移为影响CYFRA21-1、DLL4、Notch表达的主要因素(P<0.05)。结论CYFRA21-1与自身抗体DLL4、Notch在肺癌患者血清中呈现高表达状况,与患者的病理分化类型、临床分期、淋巴结转移存在相关性。

Objective To explore the expression of cytokeratin 21-1(CYFRA21-1)in the serum of lung cancer patients and the correlation between autoantibodies and clinicopathological characteristics.Methods 78 cases of lung cancer patients were selected as the lung cancer group,and 78 cases of healthy people of the same sex(proportionally matched)and similar age were selected as the healthy group during the same period,and the serum CYFRA21,DLL4 and Notchof the 2 groups were detected,and the correlation with the clinicopathological characteristics of lung cancer patients were analyzed.Results The positive rates of CYFRA21-1,DLL4 and Notch in the lung cancer group were 85.9%,83.3% and 62.8%,respectively,which were significantly higher than those in the healthy group of 5.1%,6.4% and 5.1%(P<0.05).In the lung cancer group,the differences in the positive expression rates of CYFRA21-1,DLL4,and Notch in lung cancer patients with different differentiation types,clinical stages,and lymph node metastasis compared were statistically significant(P<0.05).Pearson correlation analysis showed that the positive rates of CYFRA21-1,DLL4,Notch expression were correlated with differentiation type,clinical stage,and lymph node metastasis(P<0.05);Logistic regression analysis showed that differentiation type,clinical stage,and lymph node metastasis were the main factors that affected CYFRA21-1,DLL4 and Notch expression(P<0.05).Conclusion CYFRA21-1,autoantibodies DLL4,and Notch are highly expressed in the serum of lung cancer patients,which are correlated with patients'pathological differentiation type,clinical stage,and lymph node metastasis.
