戊酸雌二醇联合黄体酮序贯治疗功能失调性子宫出血的临床效果

Clinical effect of sequential treatment with estradiol valerate and progesterone on dysfunctional uterine bleeding

目的观察戊酸雌二醇联合黄体酮序贯治疗功能失调性子宫出血的临床效果。方法选择2016年7月-2019年7月于泉州玛珂迩妇产医院接受治疗的功能失调性子宫出血患者82例,依据患者意愿分为单药组和联合组,每组41例。单药组予以戊酸雌二醇治疗,联合组予以戊酸雌二醇联合黄体酮序贯治疗。比较2组临床疗效、出血控制时间、完全止血时间、治疗前后子宫内膜厚度及血清激素[垂体促卵泡素(FSH)、黄体生成素(LH)、雌二醇(E2)及孕酮(P)]水平。结果联合组总有效率为95.12%,高于单药组的80.49%(χ^(2)=4.100,P=0.043)。联合组出血控制时间及完全止血时间短于单药组(P<0.01)。治疗后,2组患者血清FSH、LH水平低于治疗前,血清E2、P水平及子宫内膜厚度高于治疗前,且联合组变化幅度大于单药组(P均<0.01)。结论戊酸雌二醇联合黄体酮序贯治疗功能失调性子宫出血的治疗效果较好,可有效控制患者子宫出血症状,促进子宫功能恢复,调节患者月经状况,改善血清激素水平,利于患者恢复。

Objective To analyze the clinical effect of sequential treatment of estradiol valerate and progesterone on dysfunctional uterine bleeding.Methods Selected 82 patients with dysfunctional uterine bleeding who were treated in Quanzhou Macare Women’s Hospital from July 2016 to July 2019.According to the wishes of patients,they were divided into combination group and single drug group,with 41 cases in each group.The single drug group was treated with estradiol valerate,and the combination group was treated with estradiol valerate combined with progesterone sequential treatment.Compared the clinical efficacy,bleeding control time,time to complete hemostasis,endometrial thickness before and after treatment of 2 groups,and serum hormones[pituitary follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2)and progesterone(P)]level.Results The total effective rate of the combined group was 95.12%,which was higher than 80.49%of the single drug group(χ~2=4.100,P=0.043).The bleeding control time and complete hemostasis time in the combined group were shorter than those in the single drug group(P<0.01).After treatment,the serum FSH and LH levels of the two groups were lower than those before treatment,and the serum E2,P levels and endometrial thickness were higher than those before treatment,and the combined group had greater changes than the single drug group(all P<0.01).Conclusion The sequential treatment of estradiol valerate and progesterone in dysfunction blood can improve the treatment effect,control the symptoms of uterine bleeding in patients,promote the recovery of uterine function,regulate their menstrual status,and improve the serum hormone level,which is beneficial to recovery.
