耳聋胶囊联合甲泼尼龙治疗突发性耳聋的临床研究

Clinical study of Erlong Capsules combined with methylprednisolone in treatment of sudden deafness

目的探讨耳聋胶囊联合甲泼尼龙治疗突发性耳聋的临床效果。方法选取2017年10月—2019年10月天津市泰达医院收治的82例突发性耳聋患者,随机分成对照组(41例)和治疗组(41例)。对照组患者口服甲泼尼龙片,48 mg/次,1次/d,晨起顿服,连服5 d。治疗组在对照组基础上口服耳聋胶囊,3粒/次,2次/d,连服10 d。两组连续治疗10 d。观察两组的临床疗效,比较两组受损频率平均听阈、耳鸣评分、耳闷胀感评分和血清学因子的变化情况。结果治疗后,治疗组突发性耳聋总有效率为90.2%,高于对照组(82.9%),但差异无统计学意义。治疗后,治疗组耳鸣总有效率是94.6%,显著高于对照组的75.0%(P<0.05)。治疗后,治疗组耳闷胀感总有效率是95.5%,显著高于对照组的66.7%(P<0.05)。治疗后,两组受损频率平均听阈均显著低于同组治疗前,且耳鸣评分、耳闷胀感评分均显著降低(P<0.05);治疗后,治疗组受损频率平均听阈、耳鸣评分、耳闷胀感评分均显著低于对照组(P<0.05)。治疗后,两组血清C反应蛋白(CRP)、肿瘤坏死因子(TNF)-α水平均显著低于本组治疗前水平(P<0.05);且治疗后,治疗组CRP、TNF-α水平显著低于对照组(P<0.05)。结论耳聋胶囊联合甲泼尼龙治疗突发性耳聋的总体疗效确切,能有效促进患者听力的恢复,尤其在改善患者伴随症状和机体炎症反应方面有显著优势,且安全性较好,值得临床推广应用。

Objective To investigate the clinical effect of Erlong Capsules combined with Methylprednisolone in treatment of sudden deafness. Methods A total of 82 patients with sudden deafness admitted to Tianjin TEDA Hospital from October 2017 to October 2019 were randomly divided into control group(41 cases) and treatment group(41 cases). Patients in the control group were po administered with Methylprednisolone Tablets, 48 mg/time, once daily. They took for 5 d after getting up in the morning. Patients in the treatment group were po administered with Erlong Capsules on the basis of the control group, 3 capsules/time, twice daily. The two groups were treated consecutively for 10 d. The clinical efficacy of the two groups was observed, and the changes of average hearing threshold of impaired frequency, tinnitus score, ear dullness score and serological factors were compared between the two groups. Results After treatment, the total effective rate of sudden deafness in the treatment group was 90.2%, higher than that in the control group(82.9%), but the difference was not statistically significant. After treatment, the total effective rate of tinnitus in the treatment group was 94.6%, significantly higher than 75.0% in the control group(P < 0.05). After treatment, the total effective rate of muffled feeling in the treatment group was 95.5%, which was significantly higher than that in the control group(66.7%, P < 0.05). After treatment, the average hearing threshold of impaired frequency in both groups was significantly lower than that before treatment, and tinnitus score and ear dullness score were significantly reduced(P < 0.05). After treatment, the average hearing threshold of impaired frequency, tinnitus score and ear dullness score in the treatment group were significantly lower than those in the control group(P < 0.05). After treatment, the serum levels of CRP and TNF-α in both groups were significantly lower than those before treatment(P < 0.05). After treatment, CRP and TNF-α levels in the treatment group were significantly lower than those in the control group(P < 0.05). Conclusion Erlong Capsules combined with Methylprednisolone has a definite overall effect in treatment of sudden deafness, and can effectively promote the hearing recovery of patients, especially has a significant advantage in improving patients’ accompanying symptoms and the body’s inflammatory response with a good safety, which is worthy of clinical promotion and application.
