爆震性耳聋临床表型及治疗效果与外周炎症细胞相关性分析 被引量:3

摘要 目的通过分析爆震性耳聋发生后外周血炎症细胞指标变化,研究外周炎症细胞与爆震性耳聋临床表型及治疗效果相关性。方法对14例(16耳)爆震性耳聋发生后外周血炎症细胞指标变化进行分析,比较不同疗效的爆震性耳聋患者间外周血炎症细胞指标的分布特征。结果所有患者听力损失程度为轻度68.8%、中度25.0%、重度6.3%;其听力图类型主要为陡降型56.25%、平坦型31.25%、钟型6.25%;治疗前后比较其听觉阈值结果,250、1000、2000、4000 Hz差异有统计学意义(P<0.05);62.50%的患者中性粒细胞升高,50.00%的患者淋巴细胞降低,单核细胞值正常与降低患者分别各占37.50%,嗜酸性粒细胞降低的患者超过62.50%;经治疗,9耳有效,7耳无效,不同疗效患者间外周血炎症指标间差异没有统计学意义(P>0.05)。结论本研究尚未发现外周炎症细胞变化与噪声性耳聋临床表型及治疗预后直接相关,仍需未来加大样本量进一步研究。 Objective To study the correlation between peripheral inflammatory cells and the clinical phenotype and treatment effect of explosive deafness by analyzing the changes of peripheral blood inflammatory cells after the occurrence of knock deafness.Methods The changes in peripheral blood inflammatory cell indexes of 14 patients(16 ears)with explosive deafness were analyzed,and the distribution characteristics of peripheral blood inflammatory cell indexes among patients with explosive deafness with different curative effects were compared.Results The hearing loss of all patients includes mild,moderate,and severe,accounting for 68.75%,25.00%,and 6.25%respectively;Their audiogram types are mainly steep drop,flat,and bell,accounting for 56.25%,31.25%,and 6.25%respectively;The audiology examination was completed before and after treatment,and the results of hearing threshold were compared before and after treatment.There were statistical differences between 250 Hz,1000 Hz,2000 Hz,and 4000 Hz(P<0.05);62.50%of patients had elevated neutrophils,50.00%of patients had decreased lymphocytes,37.50%of patients with normal and decreased monocyte values,and more than 62.50%of patients with decreased eosinophils;After treatment,9 ears were effective,7 ears were ineffective,and no statistical significance difference was found in the inflammation indexes of peripheral blood between patients with different curative effects(P>0.05).Conclusion This study has not found that the changes of peripheral inflammatory cells are directly related to the clinical phenotype and treatment prognosis of noise-induced deafness,and further research is needed to increase the sample size in future studies.
