不同季节人工授精助孕结局分析

Analysis of intrauterine insemination outcomes in different seasons

目的探索季节与人工授精助孕结局的相关性。方法回顾性分析2017年1月至2018年12月在西北妇女儿童医院接受第1周期夫精人工授精(AIH)助孕的原发性不孕症患者307例,根据手术日期,分为春季(n=68)、夏季(n=90)、秋季(n=82)和冬季(n=67)四组,分析不同季节人工授精助孕的结局。结果不同季节患者的年龄、体质量指数(BMI)、不孕年限、基础雌二醇(E2)、基础卵泡刺激素(FSH)、E2峰值、手术日内膜厚度及处理后前向运动精子总数差异均无统计学意义(均P>0.05)。春季、夏季、秋季和冬季的临床妊娠率分别为23.53%、15.56%、14.63%和10.45%,差异无统计学意义(χ^(2)=4.541,P>0.05);四组流产率分别为18.75%、14.29%、0、14.29%,差异无统计学意义(P>0.05);四组活产率分别为19.12%、13.33%、14.63%、8.96%,差异无统计学意义(χ^(2)=2.955,P>0.05);四组的新生儿出生体重分别为(3.29±0.49)kg、(3.39±0.41)kg、(3.30±0.59)kg和(3.54±0.31)kg,差异无统计学意义(F=0.441,P>0.05)。结论季节与人工授精助孕结局无明显相关性。

Objective To explore the relationship between season and intrauterine insemination(IUI)outcomes.Methods This retrospective study was conducted on 307 patients who were primary infertility and underwent the first natural cycles in IUI between January 2017 and December 2018 at Northwest Women’s and Children’s Hospital.According to the day of surgery,subjects were divided into four groups by calendar months:spring(n=68),summer(n=90),autumn(n=82),winter(n=67).IUI outcomes were calculated and compared among these four groups.Results There were no significant differences among the seasonal groups in terms of age,body mass index,duration of infertility,baseline FSH level,baseline E2 level,peak E2 level,endometrial thickness and sperm count with progressive mobile spermatozoa after washing on the surgery day(all P>0.05).The clinical pregnancy rates for the spring,summer,autumn and winter groups were 23.53%,15.56%,14.63% and 10.45%,respectively,and there were no significant difference(χ^(2)=4.541,P>0.05).The miscarriage rates among these four groups were 18.75%,14.29%,0 and 14.29%,respectively,and there were no significant difference(P>0.05).The live birth rates among these four groups were 19.12%,13.33%,14.63%and 8.96%,respectively,and there were no significant difference(χ^(2)=2.955,P>0.05).The newborn birth weight among these four groups were(3.29±0.49)kg,(3.39±0.41)kg,(3.30±0.59)kg and(3.54±0.31)kg,respectively,and there were no significant difference(F=0.441,P>0.05).Conclusion There were no significant difference of seasonal variability in the outcome of IUI.
