参附强心汤治疗老年重症心力衰竭临床研究

Clinical Study on Shenfu Qiangxin Tang for Severe Heart Failure in Senile Patients

目的:观察在西医常规的基础上加用参附强心汤治疗老年重症心力衰竭的临床疗效。方法:将重症心力衰竭老年患者52例随机分为2组,各26例。对照组采用单纯西医常规治疗,观察组在对照组的基础上加用参附强心汤治疗,均以7 d为1个疗程,共治疗2个疗程。比较2组临床疗效及治疗前后心功能指标左室收缩末容积(LVESV)、左心射血分数(LVEF)、每搏心输出量(SVC)、心输出量(CO)的变化情况。结果:总有效率观察组为88.46%,对照组为65.38%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组患者LVESV均较治疗前降低(P<0.05),LVEF、SVC、CO均较治疗前升高(P<0.05);且观察组各项指标改善较对照组更显著(P<0.05)。结论:参附强心汤联合西药治疗老年重症心力衰竭,可有效改善患者心功能,疗效优于单纯西药治疗。

Objective:To observe the clinical effect of Shenfu Qiangxin tang based on routine western medicine for severe heart failure in senile patients.Methods:A total of 52 cases of senile patients with severe heart failure were randomly divided into two groups,26 cases in each group.The control group was simply treated with routine western medicine,and the observation group was additionally treated with Shenfu Qiangxin tang based on the treatment of the control group.Both groups were treated for two courses,seven days being a course.The changes in the indexes of heart function including left ventricular end-systolic volume,left ventricular ejection fraction,cardiac stroke volume and cardiac output before and after treatment and the clinical effect in the two groups were observed.Results:The total effective rate was 88.46%in the observation group and 65.38%in the control group,the difference being significant(P<0.05).After treatment,the left ventricular end-systolic volumes in the two groups were decreased when compared with those before treatment(P<0.05),and the left ventricular ejection fraction,cardiac stroke volume and cardiac output were increased(P<0.05);the improvement of the above indexes in the observation group was more significant than that in the control group(P<0.05).Conclusion:The therapy of Shenfu Qiangxin tang combined with routine western medicine for severe heart failure in senile patients can effectively improve their heart function,whose curative effect is better than that of simple western medicine.
