经墨菲氏滴管注射右美托咪定用于支气管结核患者纤维支气管镜治疗的临床效果

Clinical efficacy of dexmedetomidine injection via Murph\s dropper in patients with endobronchial tuberculosis undergoing fiberoptic bronchoscope

目的观察经墨菲氏滴管注射右美托咪定在支气管结核患者纤维支气管镜(FOB)治疗中的临床效果。方法80例支气管结核患者择期行喉罩全身麻醉下实施支气管内治疗,随机均分为右美托咪定10、20、30μg组(D1、D2、D3组)和对照组(C组)。观察各组患者围术期不良反应发生情况,同时记录患者丙泊酚用量、苏醒情况及患者满意度。结果与C组相比,D3组呛咳反应发生率及严重程度降低(P<0.05),D1、D2组围术期心动过速发生率降低(P<0.05);各组围术期其他不良反应发生率差异无统计学意义(P>0.05)。与C组相比,D1、D2、D3组患者丙泊酚用量、拔管时间及离开FOB室时间稍减少,患者非常满意的比例稍增高,但差异均无统计学意义(P>0.05)。结论经墨菲氏滴管注射小剂量右美托咪定用于支气管结核患者FOB治疗简单易行,且安全有效;为此,右美托咪定30μg为较合适的剂量。

Objective To observe the clinical efficacy of dexmedetomidine injection via Murphy’s dropper in the patients with endobronchial tuberculosis undergoing fiberoptic bronchoscopy(FOB).Methods Eighty patients with endobronchial tuberculosis undergoing intrabronchial therapy under laryngeal mask-general anesthesia were equally randomized into four groups.The patients in groups of D1,D2 and D3 were injected with dexmedetomidine 10,20 and 30μg via Murphy’s dropper,respectively.The patients in group C were injected with normal saline 2 mL as the controls.The incidence of perioperative adverse responses were observed.The propofol consumption,recovery time and patients’satisfaction were also recorded.Results Compared with group C,the incidence and severity of coughing were decreased in group D3(P<0.05),and the incidence of tachycardia was decreased in groups of D1 and D2(P<0.05).But there was no statistical difference in the incidence of other adverse responses among groups of D1,D2,D3 and C(P>0.05).Compared with group C,the propofol consumption,extubation time and the time left FOB room were slightly decreased,and the patients’satisfaction degree was slightly increased in groups of D1,D2 and D3(P>0.05).ConclusionA small dose of dexmedetomidine injection via Murphy’s dropper in the patients with endobronchial tuberculosis undergoing treatment is simple,safe and effective,for which dexmedetomidine 30μg is a suitable dosage.
