医院-社区协同健康管理模式对脑卒中恢复期患者二级预防的效果

The Effect of Hospital-community Collaborative Health Management Model on Secondary Prevention of Stroke Patients in Recovery Period

目的:分析医院-社区协同健康管理模式对脑卒中恢复期患者二级预防的效果。方法:选取2017年2月-2019年6月笔者所在医院收治的86例脑卒中患者为研究对象,按入院时间先后顺序分为参照组(常规护理)和研究组(医院-社区协同健康管理模式),各43例。随访对比两组依从率、干预前后健康知识掌握度评分、生活质量评分及复发率。结果:研究组依从率90.70%,高于参照组的69.77%,差异有统计学意义(P<0.05);两组干预前,健康知识掌握度评分、生活质量评分比较差异无统计学意义(P>0.05);干预后12个月,研究组健康知识掌握度评分、生活质量评分均高于参照组,差异均有统计学意义(P<0.05);研究组12个月内复发率6.98%,低于参照组的23.26%,差异有统计学意义(P<0.05)。结论:医院-社区协同健康管理模式用于脑卒中恢复期患者二级预防中,可有效提升患者依从率及健康知识掌握度评分,对提升患者生活质量并降低复发率具有重要作用,可推广。

Objective:To analyze the effect of hospital-community collaborative health management model on secondary prevention of stroke patients in recovery period.Method:A total of 86 stroke patients admitted to our hospital from February 2017 to June 2019 were selected as the research objects.They were divided into the reference group(routine care)and the research group(hospital-community collaborative health management model)according to the order of admission time.There were 43 cases in each group.Compliance rate,health knowledge mastery score before and after intervention,quality of life score and recurrence rate of the two groups were compared.Result:The compliance rate of the study group was 90.70%,which was higher than 69.77%of the reference group,the difference was statistically significant(P<0.05);before the intervention,the two groups had no significant difference in health knowledge mastery score and quality of life score(P>0.05);12 months after the intervention,the health knowledge mastery score and quality of life score of the study group were higher than those of the reference group,and the difference was statistically significant(P<0.05);the recurrence rate of the study group within 12 months was 6.98%,which was lower than 23.26%of the reference group,the difference was statistically significant(P<0.05).Conclusion:The hospital-community collaborative health management model used in the secondary prevention of stroke recovery patients can effectively improve patient compliance rate and health knowledge mastery score,which plays an important role in improving patients’quality of life and reducing recurrence rate,and can be promoted.
