单孔腹腔镜下阴式子宫切除和传统腹腔镜全子宫切除的对比研究

A Comparative Study between Transumbilical Single-port Laparoscopic Vaginal Hysterectomy and Conventional Laparoscopic Total Hysterectomy

目的:探究经脐单孔腹腔镜下阴式子宫切除和传统腹腔镜全子宫切除在子宫肌瘤中的应用效果。方法:选取2019年10月-2020年7月阳东区人民医院收治的子宫肌瘤患者共60例为研究对象,按随机数字表法将入组患者分为观察组(n=30)和对照组(n=30)。对照组开展传统腹腔镜全子宫切除术,观察组开展经脐单孔腹腔镜下阴式子宫切除术,比较两组临床指标及术后情况。结果:观察组手术时间长于对照组,差异有统计学意义(P<0.05);观察组术中出血量虽少于对照组,但差异无统计学意义(P>0.05)。观察组术后并发症少于对照组,术后1、3 d的VAS评分低于对照组,差异有统计学意义(P<0.05);两组术后排气时间与住院时间差异无统计学意义(P>0.05)。结论:经脐单孔腹腔镜下阴式子宫切除术在子宫肌瘤中应用安全可行,虽手术时间稍长,但可减少术后并发症发生率,降低术后疼痛程度,且术后腹部无可见瘢痕,美容效果肯定。

Objective:To explore the application effect of transumbilical single-port laparoscopic vaginal hysterectomy and traditional laparoscopic total hysterectomy in hysteromyoma.Method:A total of 60 patients with uterine fibroids admitted to Yangdong District People’s Hospital from October 2019 to July 2020 were selected as study subjects.According to the random number table method,the enrolled patients were divided into the observation group(n=30)and the control group(n=30).The control group underwent traditional laparoscopic total hysterectomy,while the observation group underwent transumbilical single-port laparoscopic vaginal hysterectomy.The clinical indicators and postoperative situation of the two groups were compared.Result:The operative time of the observation group was longer than that of the control group,and the difference was statistically significant(P<0.05).The intraoperative blood loss in the observation group was less than that in the control group,but the difference was not statistically significant(P>0.05).Postoperative complications in the observation group was less than that in the control group,and scores of 1 d and 3 d VAS after surgery were lower than those in the control group,with statistically significant differences(P<0.05).Postoperative exhaust time and hospital stay time were not significantly different between the two groups,and the differences were not statistically significant(P>0.05).Conclusion:Transumbilical single-port laparoscopic vaginal hysterectomy is safe and feasible in the treatment of uterine fibroids.Although the operation time is a little longer,it can reducing the incidence of postoperative complications and reducing postoperative pain,and there is no scar on the abdomen after the operation,so the aesthetic effect is sure.
