高龄二胎产妇应用中心群组化孕期保健模式的效果及对产妇心理状态、分娩结局的影响

The Effect of the Application of the Central Group-based Health Care Model during Pregnancy for the Elder Second-born Mothers and ItsInfluence on the Mental State of the Mothers and the Outcome of Delivery

目的:分析高龄二胎产妇应用中心群组化孕期保健模式的效果及对产妇心理状态、分娩结局的影响。方法:选取2019年1-12月于本院分娩的1 000例高龄二胎产妇,将其按随机数字表法分为两组,各500例产妇,对照组实施常规护理,研究组实施中心群组化孕期保健模式护理。比较两组心理状态[焦虑自评量表(SAS)、抑郁自评量表(SDS)],产妇第一、第二、第三及总产程时间,术后2 h出血量,分娩结局,护理满意度。结果:分娩后,两组SAS、SDS评分均降低,且研究组低于对照组(P<0.05);研究组第一、第二、第三及总产程时间均低于对照组,术后2 h出血量低于对照组(P<0.05);研究组阴道分娩的占比高于对照组,难产中的剖宫产、阴道助产的占比低于对照组(P<0.05);研究组护理总满意度高于对照组(P<0.05)。结论:中心群组化孕期保健模式在高龄二胎产妇中的应用效果良好,可有效增强产妇心理素质,改善分娩结局。

Objective:To analyze the effect of central group-based health care model in advanced second-born maternity application center on maternal psychological status and delivery outcome.Method:A total of 1000 elderly second-born women who gave birth in our hospital from January to December 2019,and divided them into two groups according to a random number table,each with 500 women.The control group performed routine care,and the study group implemented central group-based health care model during pregnancy.The psychological status of the two groups[self-rating anxiety scale(SAS),self-rating depression scale(SDS)],the first,second,third,and total labor time of the parturient,the amount of bleeding 2 hours after operation,delivery outcome,nursing satisfaction in two groups were compared.Result:After delivery,the SAS and SDS scores in both groups were decreased,and those in the study group were lower than those in the control group(P<0.05).The first,second,third and total duration of Labor in the study group were lower than those in the control group,and the amount of bleeding 2 hours after operation was lower than that in the control group(P<0.05).The proportion of vaginal delivery in the study group was higher than that in the control group,and the proportion of cesarean section and induction delivery in dystocia were lower than those in the control group(P<0.05).The total nursing satisfaction of the study group was higher than that of the control group(P<0.05).Conclusion:The application of central group-based health care model in the elderly second-child pregnant women is effective.
