穴位电疗及中药外敷对DDH术后患儿胃肠功能的影响

Effects of Acupoint Electrotherapy and External Application of Traditional Chinese Medicine on Gastrointestinal Function Recovery of Children after DDH

目的:观察穴位电疗结合中药贴敷对发育性髋关节脱位(DDH)术后患儿胃肠功能的影响。方法:将120例发育性髋关节脱位患儿随机分为对照组60例、观察组60例。对照组常规予儿童骨科术后一般护理,观察组患儿在对照组的基础上,在术后半小时、术后24 h、术后48 h分别给予中频电刺激治疗仪作用于患儿天枢穴、大肠俞,治疗时间20 min,后将具有通便作用的中药外敷于患儿的中脘、天枢及气海穴,记录两组患儿手术后的胃肠功能恢复时间(肠鸣音恢复时间、术后第一次排气排便时间)、胃肠功能恢复评分、胃肠道不良反应类别及出现次数,评价两组患儿家长对护理工作在关爱患儿、业务能力、护理态度、健康宣教、心理暗示方面的满意度,并进行统计学分析。结果:观察组患儿胃肠功能恢复时间短于对照组,胃肠功能恢复评分低于对照组,出现恶心、呕吐、腹胀的胃肠道不良反应发生率低于对照组,观察组患儿家长对护理工作在关爱患儿、业务能力、护理态度、健康宣教、心理暗示方面的满意度优于对照组,差异均有统计学意义(P<0.05);两组患儿腹痛发生率比较,差异无统计学意义(P>0.05)。结论:中频脉冲电疗联合穴位贴敷能降低发育性髋关节脱位患儿出现恶心、呕吐、腹胀的胃肠道不良反应发生率,促进术后胃肠功能恢复,提高家长的满意度。

Objective:To observe the effect of acupoint electrotherapy and external application of Traditional Chinese Medicine on gastrointestinal function of children with developmental hip dislocation(DDH).Method s:Totally 120 patients with developmental dislocation of hip joint were randomly divided into control group(60 cases)and treatment group(60 cases).Control group was treated with routine care for orthopedic surgery.On the basis of the control group,the treatment group was given intermediate frequency electric stimulation in Tianshu(ST25)and Dachangshu(BL25)for 20 minute,and then treatment group was given external treatment in Zhongwan(CV12),Tianshu(ST25)and Qihai(CV6)with Traditional Chinese Medicine of relaxing bowels effect,0.5 h,24 h and 48 h after operation.Gastrointestinal function recovery time after surgery(including bowel sound recovery time and postoperative exhaustion time),gastrointestinal function recovery scale,gastrointestinal adverse reaction type and occurrences were observed.And the satisfaction of the parents on the aspects of caring for the children,professional ability,nursing attitude,health education and psychological suggestion were evaluated.Statistical analysis was conducted.Results:Treatment group showed shorter gastrointestinal function recovery time than the control group,lower gastrointestinal functional recovery score than control group,and lower incidence of gastrointestinal side effects than control group,including nausea,vomiting and abdominal distension.The parents in treatment group showed superior satisfaction than control group on caring for children,ability of nursing work,nursing attitude,health education and psychological suggests,and the difference was statistical significance(P<0.05).There was no significant difference in the incidence of abdominal pain between the two groups(P>0.05).Conclusion:Acupoint electrotherapy combined with Chinese Medicine application can reduce the incidence of gastrointestinal adverse reactions such as nausea,vomiting and abdominal distension in children with developmental hip dislocation,promote postoperative gastrointestinal function recovery,and improve the satisfaction of parents.
