针灸治疗产后尿潴留的选穴规律分析

Study on the Regularity of Acupuncture and Moxibustion for Treatment of Postpartum Urinary Retention

目的:基于现代文献分析针灸治疗产后尿潴留的选穴规律及特点。方法:以"针灸"和"产后尿潴留"等为检索词,计算机检索万方数据库、中国知网、维普数据库、中国生物医学文献数据库及PubMed,建立针灸处方数据库,采用数据挖掘技术分析选穴规律。结果:共纳入文献362篇,选用腧穴72个,总频数1 624次。使用频数最高的腧穴依次为三阴交、中极和关元;腧穴归经依次为任脉、脾经、胃经、膀胱经,未涉及心经、小肠经;腧穴主要分布在下肢部和腹部;特定穴多使用交会穴和募穴。关联分析结果显示"中极→三阴交"支持度最高,"足三里、关元、中极→三阴交"置信度最高;聚类分析可分为两大类。结论:针灸治疗产后尿潴留以三阴交、中极、阴陵泉、足三里和关元为核心穴位组合,重视循经取穴及辨证配穴。

Objective: To analyze the rules and characteristics of acupoint selection in clinical acupuncture treatment of postpartum urinary retention based on modern literature. Methods: Using "acupuncture and moxibustion" and "postpartum urinary retention" as search terms, computer searched the databases of Wanfang, CNKI,VIP, CBM and PubMed. The database of acupuncture prescriptions was established to analyze the acupoint selection rule using the data mining technique. Results: A total of 362 articles were included, 72 acupoints were selected and the total use frequency was 1 624 times. The results showed that Sanyinjiao(SP6) was used the most, followed by Zhongji(RN3) and Yinlingquan(SP9). The selected acupoints mainly belonged to conception vessel, spleen meridian, stomach meridian and bladder meridian, without involving the heart meridian and small intestine meridian. The acupoints were mainly distributed in the lower limbs and abdomen. Crossing points and front-mu points were commonly selected in the specific acupoints. The correlation analysis results show that Zhongji(RN3)→Sanyinjiao(SP6) has the highest support, and Zusanli(ST36), Guanyuan(RN4), Zhongji(RN3)→Sanyinjiao(SP6) has the highest confidence. The cluster analysis can be divided into two categories. Conclusion:The clinical selection of acupuncture and moxibustion for postpartum urinary retention mainly focuses on Sanyinjiao(SP6), Zhongji(RN3), Yinlingquan(SP9), Zusanli(ST36) and Guanyuan(RN4), with emphasis on selection of acupoints along the meridian and dialectical matching.
