中医临床护理路径对肾移植患者免疫抑制药物服药依从及代谢紊乱症的影响

The Influence of Clinical Nursing Pathway of Traditional Chinese Medicine on Medication Compliance of Immunosuppressive Drugs and Metabolic Disorders in Renal Transplant Patients

目的:探讨中医临床护理路径对肾移植患者免疫抑制药物服药依从及代谢紊乱症的应用价值。方法:选取进行肾移植手术的患者62例,按照入院顺序分为观察组及对照组,每组31例。对照组采用常规护理模式,观察组采用中医临床路径护理模式。比较两组患者干预后代谢紊乱症发生率、服药依从性评分(basel assessment of adherence with immunosuppressive medication scale,BAASIS)、生活质量及护理满意度。结果:干预后观察组患者高尿酸血症、高血压、糖尿病、血脂异常发生率低于对照组,两组比较差异有统计学意义(P<0.05);干预后观察组BAASIS评分低于对照组,(immunosuppressant therapy adherence instrument,ITAS)评分、Morisky评分高于对照组,差异均有统计学意义(P<0.05);干预后观察组躯体健康/功能、心理/精神、社会/经济、家庭4个维度生活质量评分高于对照组,差异有统计学意义(P<0.05);干预后观察组患者护理总满意率高于对照组,差异有统计学意义(P<0.05)。结论:中医临床护理路径可明显提升肾移植患者术后免疫抑制药物服药依从性,降低代谢紊乱症发生率,提高生活质量。

Objective:To explore the application value of clinical nursing pathway of traditional Chinese medicine on medication compliance of immunosuppressive drugs and metabolic disorders in renal transplant patients.Methods:A total of 62 patients who underwent kidney transplantation were selected and divided into the observation group and the control group according to the order of admission,31 cases each group.The control group adopted conventional nursing model,and the observation group clinical nursing pathway of traditional Chinese medicine.To compare the incidence of metabolic disorder,BAASIS quality of life and nursing satisfaction between both groups after the intervention.Results:After the intervention,the observation group was lower than the control group in the incidences of hyperuricemia,hypertension,diabetes mellitus and dyslipidemia,and the difference was statistically significant(P<0.05);after the intervention,the observation group was lower than the control group in BAASIS scores,higher than the control group in ITAS scores,Morisky scores,and the difference was statistically significant(P<0.05).After the intervention,the observation group was higher than the control group in the scores of quality of life including somatic health/function,psychology/spirit,society/economy and family,and the difference was statistically significant(P<0.05);after intervention,the total satisfaction rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Clinical nursing pathway of traditional Chinese medicine could notably raise medication compliance of immunosuppressive drugs,reduce the incidence of metabolic disorders in renal transplant patients,and improve quality of life.
