六味地黄丸治疗红斑狼疮的临床观察

Clinical Observation on Liuwei Dihuang Pill in the Treatment of Systemic Lupus Erythematosus

目的探究六味地黄丸治疗系统性红斑狼疮的临床效果。方法选取中国医科大学附属盛京医院收治的系统性红斑狼疮患者42例,根据随机数字表法分为对照组和试验组,各21例。2组均给予常规治疗,对照组采用血液净化装置+强的松龙注射液+环磷酰胺,试验组在对照组的基础上加服六味地黄丸。2组均以3个月为1个疗程。治疗结束后对比分析2组患者临床疗效、血清单核细胞趋化蛋白-4(MCP-4)、白细胞介素-4(IL-4)、系统性红斑狼疮疾病活动指数(SLEDAI)积分、24 h尿蛋白以及不良反应情况。结果治疗后与对照组相比,试验组临床总有效率较高(P<0.05)。治疗后2组MCP-4、IL-4、SLEDAI评分及24 h尿蛋白水平降低(P<0.05);与对照组相比,试验组MCP-4、IL-4、SLEDAI评分及24 h尿蛋白较低(P<0.05)。2组不良反应总发生率相比,差异无统计学意义(P>0.05)。结论六味地黄丸治疗系统性红斑狼疮疗效显著,能够有效降低血清IL-4及MCP-4水平,同时能够有效恢复Th1和Th2细胞免疫平衡,适宜临床应用推广。

Objective To investigate the effect of Liuwei Dihuang pill combined with cyclophosphamide on monocyte chemoattractant protein-4 and interleukin-4 in patients with systemic lupus erythematosus.Methods 42 patients with systemic lupus erythematosus in our hospital were divided into control group and study group according to the random control table,21 patients each.The control group received blood purification device plus prednisolone injection plus cyclophosphamide,and the study group received more with Liuwei Dihuang pill,2 groups were treated with a course of 3 months.After treatment,the clinical effect,serum monocyte chemoattractant protein-4(MCP-4),interleukin-4(IL-4),systemic lupus erythematosus disease activity index(SLEDAI)score,24-hour urine protein and adverse reactions were compared and analyzed.Results Compared with the control group,the total clinical effective rate of the study group was higher(P<0.05).Compared with before treatment,levels of MCP-4,IL-4,SLEDAI scores and 24-hour urinary protein levels in 2 groups decreased(P<0.05),and compared with the control group,levels of MCP-4,IL-4,SLEDAI scores and 24-hour urinary protein levels in the study group were lower(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Liuwei Dihuang pill is effective in the treatment of systemic lupus erythematosus,which can effectively reduce the levels of serum IL-4 and MCP-4,and restore the immune balance of Th1 and Th2 cells,which is suitable for clinical application.
