老年无痛胃肠镜应用地佐辛联合丙泊酚麻醉效果

Anesthesia effect of dezocine combined with propofol in elderly patients with painless gastrointestinal endoscopy

目的探讨地佐辛联合丙泊酚应用在老年无痛胃肠镜中的麻醉效果。方法选取2017年1月-2018年12月在广饶县人民医院接受无痛胃肠镜检查的82例老年患者作为研究对象,采取抽签法分为参照组和观察组各41例。参照组采用丙泊酚进行麻醉,观察组采用地佐辛联合丙泊酚进行麻醉,比较两组患者呛咳及不自主肢体活动等不良反应发生率、丙泊酚用量、血氧饱和度(SpO_(2))、血压、心率变化及唤醒时间。结果观察组患者的收缩压、舒张压、心率、SpO_(2)指标均优于参照组,差异均有统计学意义(P<0.05)。观察组丙泊酚用量明显低于参照组;观察组不良反应发生率为4.88%,明显低于参照组的24.39%,差异均有统计学意义(P<0.05)。观察组唤醒时间、定向力恢复时间均优于参照组,差异均有统计学意义(P<0.05)。结论地佐辛联合丙泊酚在老年无痛胃肠镜麻醉中应用效果良好,可有效减少患者术中呛咳及不自主肢体活动发生,减少丙泊酚用量,减轻呼吸循环抑制,缩短苏醒时间,具有临床应用及推广价值。

Objective To explore the anesthesia effect of dezocine combined with propofol in elderly patients with painless gastrointestinal endoscopy. Methods Eighty-two elderly patients who received painless gastrointestinal endoscopy test in Guangrao County People’s Hospital from January 2017 to December 2018 were selected as the research objects,and they were divided into the reference group and the observation group by drawing lots,with 41 cases in each group. The reference group was anesthetized with propofol,and the observation group was anesthetized with dezocine combined with propofol. The incidence of adverse reactions such as choking cough and involuntary limb movement,dosage of propofol,blood oxygen saturation(SpO_(2)),blood pressure and heart rate changes and wake-up time were compared between the two groups. Results The systolic blood pressure,diastolic blood pressure,heart rate and SpO_(2) of the observation group were better than those of the reference group,and the differences were statistically significant(P<0.05). The dosage of propofol in the observation group was significantly lower than that in the reference group;the incidence of adverse reactions in the observation group was 4.88%,which was significantly lower than 24.39% in the reference group,and the differences were statistically significant(P<0.05). The wake-up time and orientation recovery time in the observation group were better than those in the reference group,and the differences were statistically significant(P<0.05). Conclusion Dezocine combined with propofol in elderly patients with painless gastrointestinal endoscopy anesthesia had a good effect,could effectively reduce the incidence of choking cough and involuntary limb movement,reduce the dosage of propofol,reduce the respiratory and circulatory inhibition,shorten the recovery time,and had clinical application and promotion value.
