整合诊疗模式在社区居家老年高血压患者中的应用效果观察

Effect of integrated diagnosis and treatment model on elderly hypertensive patients at home in community

目的探索符合我国国情的社区老年高血压患者的整合诊疗模式。方法选取2019年1月—12月期间社区居家老年高血压患者200人,分为对照组和观察组,每组各100人。对照组给予常规诊疗,观察组采用整合诊疗模式。随访并比较6个月后两组患者应用效果。结果观察组在患者的生理、心理-社会指标改善情况上,明显优于对照组(P<0.05)。结论整合诊疗模式能促进血压的有效控制,值得临床推广和应用。

Objective To explore the community’s elderly hypertension patients’ integrated diagnosis and treatment model in line with China’s national conditions.Methods Two hundred selected elderly patients with hypertensive from January 2019 to December 2019 were divided into the study group and control group, with 100 patients in each. The control group received routine diagnosis and treatment while the study group the integrated diagnosis and treatment model. The application effect of two groups after 6 months of follow-up was compared.Results The physiological, psychological and social indicators of the study group performed significantly better than that of the control group(P<0.05).Conclusion The integrated diagnosis and treatment model promotes an effective control of blood pressure, thus it is worthy of clinical promotion and application.
