摘要
目的探讨首发青少年抑郁症患者认知功能与细胞因子的关系。方法抽取2016年7月至2018年7月就诊于郑州大学第一附属医院门诊首发且未用药的青少年抑郁症患者75例为抑郁症组,另抽取年龄、性别和受教育年限相匹配的健康者75例为对照组。采用酶联免疫吸附试验测定受试者细胞因子水平,应用认知共识测验评定认知功能,比较两组受试者细胞因子水平和认知功能,并行Pearson相关性分析。结果抑郁症组信息处理速度、注意或警觉性、工作记忆、词语学习、视觉学习、推理和问题解决、社会认知评分低于对照组(t=-6.740、-5.623、-6.605、-4.492、-7.441、-6.839、-6.990,P<0.05);抑郁症组血清白细胞介素-2、白细胞介素-6(IL-6)、白细胞介素-1β和肿瘤坏死因子-α(TNF-α)水平高于对照组(t=4.982、2.656、7.195、10.322,P<0.05)。抑郁症组患者工作记忆、推理和问题解决、社会认知评分与TNF-α水平呈负相关(r=-0.385、-0.237、-0.250,P<0.05),词语学习和视觉学习评分与IL-6水平呈负相关(r=-0.243、-0.252,P<0.05)。结论首发青少年抑郁症患者存在细胞因子水平升高、认知功能广泛受损,TNF-α水平越高者其工作记忆、推理和问题解决、社会认知受损越明显,IL-6水平越高者词语学习和视觉学习受损越明显。
Objective To investigate the relationship between cognitive function and cytokines in first-episode adolescent depression.Methods Seventy-five patients with first-episode drug-free adolescent depressive who were diagnosed at the outpatient clinic of the First Affiliated Hospital of Zhengzhou University from July 2016 to July 2018 were selected as the depression group.In addition,75 healthy with matching age,gender and years of education were selected as the control group.Enzyme-linked immunosorbent assay was used to determine the level of cytokines in subjects,and cognitive function was assessed by consensus cognitive battery test.The cytokine levels and cognitive functions of the two groups of subjects were compared,and Pearson correlation analysis was performed.Results The scores of speed of information processing,attention or alertness,working memory,word learning,visual learning,reasoning and problem solving,and social cognitive function in the depression group were lower than those in the control group(t=-6.740,-5.623,-6.605,-4.492,-7.441,-6.839,-6.990,P<0.05).The levels of serum interleukin-2,interleukin-6(IL-6),interleukin-1βand tumor necrosis factor-α(TNF-α)in the depression group were higher than those in the control group(t=4.982,2.656,7.195,10.322,P<0.05).The working memory,reasoning and problem solving,social cognitive scores of patients in the depression group were negatively correlated with TNF-αlevels(r=-0.385,-0.237,-0.250,P<0.05),word learning and visual learning scores were correlated with the IL-6 level was negatively correlated(r=-0.243,-0.252,P<0.05).Conclusions The level of cytokines is increased and cognitive function is impaired in the first episode of adolescent depression.The level of TNF-αwere higher,the impairment of working memory,reasoning,problem solving and social cognition were more obvious.The level of IL-6 were higher,the impairment of word learning and visual learning were more obvious.
