摘要
目的探讨CT曲面重建成像对阑尾炎的诊断价值。方法抽取2019年3月至2020年3月大同市第五人民医院收治的高度疑似阑尾炎的患者98例,均行CT增强检查并经曲面重建成像后处理。比较CT增强、曲面重建成像对阑尾炎的诊断效能及影像学征象显示情况。结果 98例高度疑似阑尾炎患者经手术确诊阑尾炎76例(77.55%)。曲面重建成像对阑尾炎的诊断敏感度为94.74%(72/76),特异度为95.45%(21/22),准确度为94.90%(93/98),分别高于CT增强的75.00%(57/76)、63.64%(14/22)、72.45%(71/98),差异有统计学意义(χ^(2)=13.606、2.718、20.271,P<0.05)。曲面重建成像对外径增粗、阑尾穿孔、阑尾结石、回盲肠壁增厚征象的显示率高于CT增强(χ^(2)=5.450、5.645、10.285、5.450,P=0.020、0.018、0.001、0.020)。结论 CT曲面重建成像可有效提升CT征象显示率,可直观显示阑尾与周围结构之间关系,对诊断阑尾炎具有较高敏感度、特异度、准确度。
Objective To investigate the clinical value of computed tomography(CT)curved surface reconstruction imaging in the diagnosis of appendicitis.Methods A total of 98 patients with highly suspected appendicitis who were admitted to Datong Fifth People’s Hospital from March 2019 to March 2020 were selected.All of them underwent enhanced CT examination and processed with curved surface reconstruction imaging.The diagnostic efficiency of CT enhancement and curved surface reconstruction imaging for appendicitis and the display of imaging signs were compared.Results Seventy-six(77.55%)of 98 patients with highly suspected appendicitis were confirmed by operation.The diagnostic sensitivity of curved surface reconstruction imaging for appendicitis was 94.74%(72/76),the specificity was 95.45%(21/22),and the accuracy was 94.90%(93/98),which were higher than 75.00%(57/76),63.64%(14/22),72.45%(71/98)of CT enhancement,with significant differences(χ^(2)=13.606,2.718,20.271,P<0.05).Curved reconstruction imaging showed higher signs of outer diameter increase,appendix perforation,appendix calculi,and ileocecal wall thickening than those of CT enhancement(χ^(2)=5.450,5.645,10.285,5.450,P=0.020,0.018,0.001,0.020).Conclusions CT curved surface reconstruction imaging can effectively improve the display rate of CT signs,can directly display the relationship between the appendix and surrounding structures,and has high sensitivity,specificity and accuracy to appendicitis.
