TEF、心率变异性与左心室射血分数保留心力衰竭患者心功能分级的相关性分析

Correlation analysis of TEF, heart rate variability and cardiac function gradingin patients with heart failure with preserved left ventricularejection fraction

目的分析TEF、心率变异性与左心室射血分数保留心力衰竭(HFpEF)患者心功能分级的相关性。方法纳入2017年1月至2019年12月我院收治的248例HFpEF患者作为HFpEF组,纳入同期入院的80例非心脏疾病患者作为对照组。比较两组的TEF和心率变异性指标;比较不同心功能分级HFpEF患者的TEF及心率变异性指标;分析HFpEF患者心功能分级与TEF及心率变异性指标的相关性,并使用ROC曲线分析TEF、心率变异性指标对HFpEF患者心功能的诊断价值。结果HFpEF组的TEF、SDANN、RMSSD和SDNN均明显小于对照组(P<0.05)。随着HFpEF患者心功能级别的增加,TEF、SDANN、RMSSD和SDN均明显降低(P<0.05)。TEF、SDANN、RMSSD和SDNN与HFpEF患者心功能分级均具有明显的相关性(P<0.05)。曲线下面积(AUC):TEF>SDNN>SDANN>RMSSD。结论TEF、心率变异性指标均与HFpEF患者心功能分级具有相关性,其中TEF的AUC大于心率变异性指标,其可能更加有利于对HFpEF患者心功能损伤的早期诊断。

Objective To analyze the correlation of TEF,heart rate variability and cardiac function grading in patients with heart failure with preserved left ventricular ejection fraction(HFpEF).Methods A total of 248 patients with HFpEF admitted in our hospital from January 2017 to December 2019 were included as HFpEF group,and 80 patients with non-cardiac disease at the same period were included as control group.The TEF and heart rate variability indexes of the two groups were compared;the TEF and heart rate variability indexes of the patients with different cardiac function gradings were compared;the correlation between the cardiac function gradings of HFpEF patients and the TEF,heart rate variability indexes were analyzed,and the diagnostic value of TEF and heart rate variability indexes for the cardiac function of HFPEF patients were analyzed by ROC curve.Results The TEF,SDANN,RMSSD and SDNN of the HFpEF group were significantly lower than those of the control group(P<0.05).TEF,SDANN,RMSSD and SDN decreased significantly with the increase of heart function grading in HFpEF patients(P<0.05).TEF,SDANN,RMSSD and SDNN were significantly correlated with the cardiac function grading of HFpEF patients(P<0.05).Area under curve(AUC):TEF>SDNN>SDANN>RMSSD.Conclusion TEF and heart rate variability indexes are all related to the heart function grading of HFpEF patients.The AUC of TEF is bigger than heart rate variability indexes,which may be more conducive to the early diagnosis of heart function injury of HFpEF patients.
