日照口岸出境人员麻疹抗体水平监测

Survey of measles antibody level among exit personnel at Rizhao port

目的调查日照口岸出境人员麻疹抗体水平,为旅行医学相关防控策略提供科学依据。方法收集2019—2020年日照口岸16岁及以上出境人员的血清,采用酶联免疫吸附法(ELISA)检测麻疹IgG抗体,对结果进行统计分析。结果日照口岸470名出境人员麻疹IgG抗体总阳性率为88.72%,抗体保护率为37.87%,抗体几何平均浓度(geometric mean concentration,GMC)为1093.57 mIU/ml。男女之间麻疹抗体阳性率、抗体保护率、GMC的差异均无统计学意义;不同年龄组之间的抗体阳性率、抗体保护率以及GMC差异均有统计学意义(P<0.05),26~<35岁人员抗体阳性率最低(86.72%)、36~<45岁最高(91.30%);抗体保护率16~<25岁最低(33.88%),46~<55岁最高(58.21%);26~<35岁人员的GMC最低(918.92 mIU/ml),46~<55岁最高(1982.18 mIU/ml);不同职业之间的抗体阳性率差异无统计学意义,船员抗体保护率最高(48.70%);留学生GMC最低(800.72 mIU/ml);2020年抗体阳性率最高(91.20%),2019年GMC最高(1306.76 mIU/ml)。结论日照口岸出境人员的麻疹免疫水平较低,国际旅行过程中麻疹发病风险较高,要加强该人群的麻疹防控工作,尤其要加强35岁及以下留学、劳务人员的麻疹疫苗接种管理以及抗体水平监测。

Objective To investigate the measles antibody level of exit personnel at Rizhao port,to provide scientific basis for the prevention and control strategies of travel medicine.Methods The Serum samples were collected from exit personnel aged 16 and older in Rizhao port from 2019 to 2020.IgG antibody against measles was detected by using enzyme-linked immunosorbent assay(ELISA),and the data was analyzed statistically.Results The general positive rate of measles IgG in 470 outbound personnel at Rizhao port was 88.72%,the antibody protection rate was37.87%,and the geometric mean concentration(GMC)was 1093.57 m IU/ml.There was no statistically significant difference in measles antibody positive rate,antibody protection rate or GMC between male and female.There was statistically significant difference of antibody positive rate,antibody protection rate and GMC between different age group(P<0.05).The positivity rate of measles antibody was lowest at 26-<35 age group(86.72%),and highest at 36-<45 age group(91.30%).The protection rate of measles antibody in 16-<25 age group(33.88%)was the lowest,in 46-<55 age group(58.21%)was the highest.The GMC was lowest at 26-<35 age group(918.92 mIU/ml),and highest at46-<55 age group(1982.18 mIU/ml).There was no significant difference in antibody positive rate between different occupation,the antibody protection rate(48.70%)was highest in the seamen and lowest in the labor workers,the GMC of abroad students was lowest(800.72 mIU/ml).The positive rate was high in 2020(91.20%),the GMC was high in 2019(1306.76 mIU/ml).Conclusion The measles immunization level of exit personnel at Rizhao port was low,and the risk of measles infection during international travel was high.It is necessary to strengthen the prevention and control of measles for the exit personnel,especially to strengthen the measles vaccination management and antibody level surveillance for the overseas students and labor aged below 35 year-old.
