不同途径梅毒感染患者临床特点及感染控制

Clinical characteristics and infection control of syphilis patients with different transmission

摘要 目的探讨院内经血传播梅毒患者与其他传播途径患者之间临床特点及梅毒患者透析感染控制。方法回顾性分析2008年7月1日—2019年8月31日在地坛医院行血液净化治疗合并梅毒血清学阳性患者的人口学和临床特征,按照梅毒传播途径分组,比较两组患者的临床特点。结果30例患者中经血传播5例,其他途径传播25例。发现梅毒血清学阳性的年龄差异无统计学差异(P=0.347)。经血传播组均为经血液透析传播,临床均表现为隐性梅毒;非经血传播组传播途径及临床表现多样。经血传播组的透析龄为(109.8±25.3)月,非经血传播组为(55.0±37.0)月,差异有统计学意义(P=0.004)。透析中心梅毒区域出现2例交叉感染,未出现梅毒与其他区域患者交叉感染。结论血液净化治疗是透析患者的感染渠道,定期监测梅毒血清学指标至关重要。 Objective To explore the clinical characteristics of syphilis patients with blood transmission and other transmission,and analyze the infection control.Methods The demographic and clinical characteristics of the hemodialysis patients with serological positive syphilis treated in the hemodialysis center of Ditan Hospital from July1,2008 to August 31,2019 were retrospectively analyzed.The patients were grouped according to the transmission,and the clinical characteristics of the two groups were compared.Results Among the 30 cases,5 cases were blood transmission and 25 cases were transmitted by other routes.There was no statistically significant difference in the age of serological positive syphilis(P=0.347).The blood transmission group was transmitted through hemodialysis,and the clinical manifestation was recessive syphilis.The transmission routes and clinical manifestations of non-blood transmission group were diverse.The age of dialysis was(109.8±25.3)months in the blood transmission group and(55.0±37.0)months in the non-blood transmission group,and the difference was statistically significant(P=0.004).There were2 cases of cross-infection in the area for syphilis patiens of dialysis center,but no cross-infection happened between syphilis and patients in other areas.Conclusion Hemodialysis therapy is the infection route of dialysis patients,and regular monitoring of syphilis serological indexes is very important.
