坐浴散熏蒸坐浴治疗混合痔术后疼痛、水肿临床研究

Clinical Study on Chinese Herbal Fumigation and Hip Bath with Zuoyu Powder forPain and Edema After Operation for Mixed Hemorrhoids

目的:观察中药坐浴散熏蒸坐浴治疗混合痔术后疼痛、水肿的临床疗效。方法:选取165例混合痔术后患者,按随机数字表法分为治疗组83例和对照组82例。2组手术方式均为外剥内扎术或痔上黏膜环切术(PPH)加外剥内扎术。术后治疗组给予坐浴散熏蒸坐浴治疗,对照组给予1∶5000高锰酸钾溶液熏蒸坐浴治疗。比较2组术后不同时间段的视觉模拟评分法(VAS)评分、肛缘水肿评分,比较2组临床疗效及术后切口愈合时间。结果:2组术后第3天、第7天,治疗组VAS评分及肛缘水肿评分均低于同期对照组,差异有统计学意义(P<0.05)。治疗组总有效率为96.38%,对照组总有效率为86.58%,2组比较,差异有统计学意义(P<0.05)。治疗组术后切口平均愈合时间短于对照组,差异有统计学意义(P<0.05)。结论:坐浴散熏蒸坐浴用于混合痔术后患者,可有效减轻其疼痛和肛缘水肿情况,促进手术切口愈合,疗效明显优于对照组。

Objective:To observe the clinical effect of Chinese herbal fumigation and hip bath with Zuoyu powder on pain and edema after operation for mixed hemorrhoids.Methods:A total of 165 patients after operation for mixed hemorrhoids were divided into the treatment group(83 cases)and the control group(82 cases)according to the random number table method.The operation methods of both groups were Milligan-Morgan hemorrhoidectomy or procedure for prolapse and hemorrhoids(PPH)plus Milligan-Morgan hemorrhoidectomy.After operation,the treatment group was given Chinese herbal fumigation and hip bath with Zuoyu powder,while the control group was given fumigation and hip bath with 1∶5000 potassium permanganate solution.The scores of Visual Analogue Scale(VAS)and perianal edema at different time points after operation were compared between the two groups.The clinical effect and postoperative wound healing time in the two groups were compared.Results:The scores of VAS and perianal edema at the same time point in the treatment group were lower than those in the control group on the third and seventh day after operation,the difference being significant(P<0.05).The total effective rate was 96.38%in the treatment group and 86.58%in the control group,the difference being significant(P<0.05).The average postoperative wound healing time in the treatment group was shorter than that in the control group,the difference being significant(P<0.05).Conclusion:Chinese herbal fumigation and hip bath with Zuoyu powder for patients with pain after operation for mixed hemorrhoids,can effectively reduce the pain and perianal edema and promote the healing of surgical wound,whose curative effect is significantly better than that of the control group.
