摘要
目的探讨早孕期母体Omentin-1、Betatrophin水平与妊娠期糖尿病(GDM)的相关性及预测价值。方法回顾性选择2016年5月至2017年5月西安医学院附属西安市北方医院妇产科接诊的53例GDM患者(GDM组),同期50例糖耐量正常孕妇(NGT组)和50例健康未孕妇女(对照组)为研究对象。酶联免疫吸附试验检测各组血清Omentin-1、Betatrophin的水平和糖脂代谢指标的差异,分析Omentin-1、Betatrophin与糖代谢指标的关系。结果血清Omentin-1、HDL-C组间对比:GDM组<NGT组<对照组(P<0.05)。血清Betatrophin与TG、孕期体重增加、新生儿出生体重呈正相关(r=0.425~0.621,P<0.05),Omentin-1与LDL-C呈正相关(r=0.509,P<0.05),与FPG、FINS、HOMA-IR、TG呈负相关(r=-0.303~0-0.592,P<0.05)。Logostic多元逐步回归分析Omentin-1(OR=2.910,P=0.000)、Betatrophin(OR=5.217,P=0.001)、BMI(OR=2.787,P=0.006)是影响GDM发病的危险因素。结论Omentin-1、Betatrophin水平变化与GDM发病有关,孕早期监测其水平可为预防GDM发生提供可靠依据。
Objective To investigate the correlation between maternal Omentin-1 and Betatrophin levels with gestational diabetes mellitus(GDM)in early pregnancy.And analyze the predictive value of Omentin-1 and Betatrophin for GDM.Methods Fifty-three patients with GDM from the perinatal health care center of our hospital from May 2016 to May 2017 were retrospectively selected as the GDM group.In the same period,50 normal pregnant women and 50 healthy pregnant women were selected as NGT group and control group.The serum levels of Omentin-1,Betatrophin and glycolipid metabolic indices were detected by ELISA.The relationship between Omentin-1,Betatrophin and glycometabolic indices were analyzed.The predictive value of Omentin-1 and Betatrophin for the occurrence of GDM were analyzed by ROC curve.Results The gestational weight gain and birth weight of newborns in GDM group were higher than those in NGT group(P<0.05).The levels of serum Betatrophin,FPG,FINS,HOMA-IR,TG and LDL-C were compared and showed:GDM group>NGT group>control group(P<0.05),and serum Omentin-1,HDL-C:GDM group<NGT group<control group(P<0.05).Serum Betatrophin was positively correlated with TG,gestational weight gain and neonatal birth weight(r=0.425-0.621,P<0.05),Omentin-1 was positively correlated with LDL-C(r=0.509,P<0.05),and negatively correlated with FPG,FINS,HOMA-IR and TG(r=-0.303-0.592,P<0.05).Logostic multiple stepwise regression analysis showed that Omentin-1(OR=2.910,P=0.000),Betatrophin(OR=5.217,P=0.001),BMI(OR=2.787,P=0.006)were risk factors for GDM.Conclusions The serum levels of Betatrophin increased and Omentin-1 decreased in the early stage of GDM.The changes of Omentin-1 and Betatrophin levels are related to the incidence of GDM.Monitoring the levels of Betatrophin in the early stage of pregnancy can provide a reliable basis for preventing the occurrence of GDM.
