甲状腺癌患者术后症状调查及其影响因素分析

Postoperative symptoms and their influencing factors in patients with thyroid cancer

目的调查甲状腺癌患者术后症状的发生率并探讨其影响因素。方法选取2019年6月至9月间北京协和医学院肿瘤医院收治的160例甲状腺癌根治术后患者,采用一般资料问卷和甲状腺癌术后症状调查表对患者症状进行问卷调查,并对调查结果进行相关影响因素分析。结果患者术后症状发生率高的前三位为疼痛、睡眠不安和咳嗽,其中≥5分的前三位症状为睡眠不安、咽痛和咳嗽。患者年龄和是否留置引流管与疼痛症状发生相关(均P<0.05),患者性别、手术方式和是否留置引流管与手足麻木症状发生相关(均P<0.05)。logistic回归分析显示,年龄和引流管为影响疼痛的因素,性别、手术方式和引流管为手足麻木的影响因素。结论甲状腺癌患者术后症状较多,了解其发生情况及相关影响因素,可指导医护人员进行针对性观察和预防性处理,改善患者术后舒适度,促进快速康复。

Objective To investigate the incidence of postoperative symptoms and analyze their influencing factors in patients with thyroid cancer.Methods One hundred and sixty patients who underwent radical resection for thyroid cancer at Peking Union Medical College Cancer Hospital between June 2019 and September 2019 were selected.A general information questionnaire and a questionnaire for postoperative symptoms after radical resection of thyroid cancer were used for the survey.Factors influencing postoperative symptoms were analyzed.Results The top three postoperative symptoms after surgery were pain,disturbed sleep,and cough,and the top three postoperative symptoms with score≥5 were disturbed sleep,sore throat and cough.Age and drainage tube indwelling state were significantly associated with pain,and gender,the choice of operation and drainage tube indwelling state were significantly associated with numbness of hands and feet(all P<0.05).Logistic regression analysis showed that age and drainage tube indwelling state were factors influencing pain and gender,the choice of operation and drainage tube indwelling state were factors influencing numbness of hands and feet.Conclusion Patients may suffer from various postoperative adverse symptoms.Understanding the incidence and influencing factors of postoperative symptoms could guide careful observation and personalized preventive treatment,improve patient comfort and promote rapid recovery.
