类风湿关节炎患者血清CEA和CA199水平与恶性肿瘤的相关性研究

Relationship between serum CEA and CA199 levels and malignant tumors in patients with rheumatoid arthritis

目的探讨类风湿关节炎患者血清癌胚抗原(CEA)和糖类抗原199(CA199)水平与恶性肿瘤的相关性。方法选取2017年12月至2019年12月间汉中市中心医院收治的48例类风湿关节炎患者为观察组,选取48例健康体检者为对照组。均行血清CEA和CA199检测,比较两组受试者恶性肿瘤发生率,比较检测结果水平及阳性率,分析观察组患者血清CEA和CA199与血沉(ESR)、C反应蛋白(CRP)、类风湿因子(RF)和抗环胍氨酸抗体(CCP)的相关性。结果观察组患者恶性肿瘤发生率为2.1%(1例),对照组为0.0%(0例),两组比较,差异无统计学意义(P>0.05)。观察组患者CEA和CA199水平均高于对照组,差异均有统计学意义(均P<0.05)。观察组患者CEA和CA199阳性率分别为14.6%(7例)和16.7%(8例),均高于对照组的2.1%(1例)和2.1%(1例),差异均有统计学意义(均P<0.05)。观察组患者血清CEA和CA199与ESR、CRP、RF和CCP均呈正相关(P<0.05)。结论类风湿关节炎患者血清CEA和CA199水平较高,阳性率高,但与恶性肿瘤无明显相关性,与疾病活动度有关,可作为评估病情进展的参考指标。

Objective To investigate the relationship between serum carcinoembryonic antigen(CEA)and carbohydrate antigen 199(CA199)levels and malignant tumors in patients with rheumatoid arthritis.Methods Forty-eight patients with rheumatoid arthritis admitted to Hanzhong Central Hospital were selected as an observation group and 48 healthy people who received physical examination were selected as a control group from December 2017 to December 2019.Serum CEA and CA199 were tested and observed in all the participants.The incidence of malignant tumors was compared between the two groups with the test result and positive rates compared.Relationship between serum CEA and CA199 and erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF)and anti-cyclic citrullinated peptide antibody(CCP)were analyzed.Results The incidence of malignant tumors was 2.1%in the observation group and 0.0%in the control group(P>0.05).The CEA and CA199 levels were significantly higher in the observation group than in the control group(all P<0.05).The positive rate of CEA and CA199 was14.6%(7 patients)and 16.7%(8 patients),respectively in the observation group which were significantly higher than 2.1%and 2.1%,respectively of the control group(all P<0.05).Serum CEA and CA199 were positively associated with ESR,CRP,RF and CCP in the observation group(all P<0.05).Conclusion Serum CEA and CA199 levels and their positive rates were high in rheumatoid arthritis They were not associated with malignant tumors.Instead they were associated with disease activity.They can be used as a reference index to predict disease pregression.
