四物汤物质基础、药理作用及临床应用研究进展

Study Progress on Compounds,Pharmacological Action and Clinical Application of Siwu Decoction

四物汤是唐朝蔺道人《仙授理伤续断秘方》中记载的经典方药,由当归、熟地黄、川芎和白芍四味中药组成,应用广泛,具有较高的临床价值。有补血活血之功,临床主要用于血虚及放化疗后血虚、再生障碍性贫血、血瘀证、骨折及术后失血、月经不调、功能性子宫出血、原发性痛经、绝经后骨质疏松等疾病,且对上述症状具有较好的临床疗效。其临床功效主要通过参与调节骨髓造血、血液流变、发挥雌激素样作用等实现,含有并鉴定了100余种成分,是其发挥药效活性的重要物质基础。四物汤代谢组学、蛋白质组学和网络药理学等系统生物学的研究为阐明其潜在作用机制,扩大临床应用提供了科学依据。本文通过对四物汤的物质基础、药理作用和临床应用研究进行系统梳理和综述,为其质量控制和深入研究提供参考。

Siwu Decoction is a classical prescription in Lin Taoist’s Xianshoulishangxuduanmifang,which is composed of four traditional Chinese medicines:Angelica sinensis(Oliv.)Diels,Ligusticum chuanxiong Hort.,Cynanchum otophyllum Schneid.and Rehmanniae radix preparata.It has been widely applied in clinic and has a good value.It mainly has Huoxue and Buxue effects,which have the obvious clinical effect on Xuexu syndrome,Xuexu syndrome after radiotherapy and chemotherapy,aplastic anemia,Xuyu syndrome,the facture,the postoperative bleeding,the irregular menstruation,the functional uterine bleeding,the primary dysmenorrhea,the postmenopausal osteoporosis and other diseases.The clinical efficacy of Siwu Decoction is mainly achieved by regulating the bone marrow hematopoiesis,the blood rheology,and exerting estrogen-like effects.In Siwu decoction,more than 100 ingredients were identified,which is an important material basis for its activity.The research of system biology of Siwu Decoction,such as metabolomics,proteomics and network pharmacology,provides a scientific basis for elucidating its potential mechanism of action and expanding clinical application.This article reviewed the material basis,pharmacological effects and the clinical application of Siwu Decoction to provide a reference for its quality control and in-depth research.
