孕早期超声检查胎儿颈项透明层增厚及静脉导管血流频谱异常在胎儿先天性心脏病筛查中的价值 被引量:17

摘要 目的分析探讨胎儿先天性心脏病筛查中孕早期超声检查胎儿颈项透明层(NT)厚度及静脉导管(DV)血流频谱异常的天性心脏病筛查中的应用价值。方法将60例健康新生儿及30例先天性心脏病新生儿作为观察对象,以健康新生儿为健康组,以先天性心脏病新生儿作为观察组,对两组新生儿孕早期胎儿NT厚度及DV血流频谱检查结果进行分析,对比两组孕早期胎儿NT厚度及DV血流频谱差异,并探讨孕早期胎儿NT厚度及DV血流频谱检查在胎儿先天性心脏病筛查中的预测价值。结果①观察组NT值、S/A值、PLI指数、PVIV指数及PIV指数均高于健康组,差异存在统计学意义(P<0.05)。②NT值、S/A值、PLI指数、PVIV指数及PIV指数对胎儿先天性心脏病的最佳预测界值分别为3.08mm、3.53、0.58、0.68及0.70。③NT值、S/A值、PLI指数、PVIV指数及PIV指数在胎儿先天性心脏病的筛查中均存在显著的预测价值(P<0.05)。结论在胎儿先天性心脏病筛查中,孕早期超声检查胎儿NT厚度及DV血流频谱检查具有显著的预测价值,临床值得应用。 Objective:To analyze and discuss the application value of ultrasonography in early pregnancy in fetal congenital heart disease screening for congenital heart disease in congenital heart disease screening of fetal neck transparent layer thickening(NT)and venous catheter(DV)abnormal blood flow spectrum.Methods:60 cases of healthy newborn and 30 cases of congenital heart disease neonates were as research object.The healthy newborns were taken as the healthy group and the newborns with congenital heart disease as the observation group.The results of NT and DV blood flow spectrum in two groups of newborns in early pregnancy were analyzed.The difference of blood flow spectrum of NT and DV in early gestation was compared between the two groups,and the predictive value of NT and DV blood flow spectrum in the screening of fetal congenital heart disease was discussed.Results:①The NT value,S/A value,PLI index,PVIV index and PIV index in the observation group were all higher than those in the healthy group,and the differences were statistically significant(P<0.05).②The best predictive thresholds of NT value,S/A value,PLI index,PVIV index and PIV index for fetal congenital heart disease were 3.08mm,3.53,0.58,0.68 and 0.70 respectively.③NT value,S/A value,PLI index,PVIV index and PIV index had significant predictive value in the screening of fetal congenital heart disease(P<0.05).Conclusion:In the screening of fetal congenital heart disease,ultrasound examination of fetal NT and DV blood flow spectrum in early pregnancy has significant predictive value,which is worthy of clinical application.
