他克莫司软膏联合蒲地蓝口服液治疗糜烂型口腔扁平苔藓患者的疗效及对免疫功能的影响

Efficacy of Tacrolimus Ointment Combined with Pudilan Oral Liquid in the Treatment of Patients with Erosive Oral Lichen Planus and Its Effect on Immune Function

目的探讨他克莫司软膏联合蒲地蓝口服液治疗糜烂型口腔扁平苔藓(OLP)患者的临床效果及对免疫功能的影响。方法选取2018年3月至2019年12月鞍山市中心医院收治的82例糜烂型OLP患者作为研究对象,按随机数字表法分为对照组与观察组,每组41例。对照组病变部位给予他克莫司软膏治疗,观察组在对照组基础上联合蒲地蓝口服液进行治疗。比较两组临床疗效,治疗前、治疗2周后视觉模拟评分法(VAS)和体征评分、免疫功能指标(CD3^(+)、CD4^(+)/CD8^(+)),及治疗后4个月患者复发情况。结果观察组治疗有效率为92.68%,明显高于对照组的75.61%,差异有统计学意义(P<0.05)。治疗前,两组VAS和体征评分比较差异无统计学意义(P>0.05);治疗后,两组VAS和体征评分降低,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组CD3^(+)和CD4^(+)/CD8^(+)比较差异无统计学意义(P>0.05);治疗后,两组CD3^(+)和CD4^(+)/CD8^(+)升高,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组复发率明显低于对照组,差异有统计学意义(P<0.05)。结论他克莫司软膏联合蒲地蓝口服液治疗糜烂型OLP疗效确切,可有效缓解患者临床症状,改善免疫功能,并减少复发情况。

Objective To investigate the clinical effect of tacrolimus ointment combined with Pudilan Oral Liquid in the treatment of erosive oral lichen planus(OLP)and its effect on immune function.Methods A total of 82 patients with erosive OLP admitted to Anshan Central Hospital from March 2018 to December 2019 were assigned into a control group and an observation group according to random number table method with 41 patients in each group.The lesions of the control group were treated with tacrolimus ointment,and the observation group was treated with Pudilan Oral Liquid plus tacrolimus ointment.The visual pain analogue scale(VAS)and physical signs scores,immune function indicators(CD3^(+),CD4^(+)/CD8^(+)),and recurrence at 4 months after treatment were compared between the 2 groups before and after 2 weeks of treatment.Results The total effective rate of the observation group was 92.68%,which was significantly higher than that of the control group(75.61%).The difference was statistically significant(P<0.05).Before treatment,there was no significant difference in VAS scores and signs scores between the 2 groups(P>0.05).After treatment,VAS scores and signs scores of the 2 group were decreased.And the observation group is lower than the control group,the difference is statistically significant(P<0.05).Before treatment,there was no significant difference in CD3^(+)and CD4^(+)/CD8^(+)between the 2 groups(P>0.05).After treatment,CD3^(+)and CD4^(+)/CD8^(+)were significantly increased in the 2 groups,and the observation group was higher than the control group(P<0.05).The recurrence rate of observation group was significantly lower than that of control group,the difference is statistically significant(P<0.05).Conclusion Tacrolimus ointment combined with Pudilan Oral Liquid is effective in the treatment of erosive OLP,which can effectively relieve clinical symptoms,improve immune function,and reduce recurrence.
