缬沙坦氨氯地平片降压治疗对老年高血压患者血压变异性的影响

Effect of Valsartan and Amlodipine Tablets on Blood Pressure Variability in Elderly Patients with Hypertension

目的探讨缬沙坦氨氯地平片降压治疗对老年高血压患者血压变异性的影响。方法选取2018年2月至2019年2月惠东县人民医院收治的183例高血压患者为研究对象,根据随机数字表法分为氨氯地平组、缬沙坦组和联合治疗组,每组61例,治疗6个月后,比较3组治疗前后血压变化、血压变异情况以及临床疗效、治疗过程中不良反应发生情况。结果治疗前3组血压和血压变异性比较差异无统计学意义(P>0.05)。治疗后3组患者白昼收缩压/舒张压、夜间收缩压/舒张压、晨峰收缩压/舒张压有所下降,且联合治疗组血压指标显著低于氨氯地平组和缬沙坦组(P<0.05),而氨氯地平组与缬沙坦组比较差异无统计学意义(P>0.05);3组24 h平均收缩压、24 h平均舒张压显著降低(P<0.05),且联合治疗组显著低于氨氯地平组和缬沙坦组(P<0.05),氨氯地平组和缬沙坦组24 h平均舒张压比较差异无统计学意义(P>0.05);3组24 h收缩压变异系数、24 h舒张压变异系数显著降低(P<0.05),且联合治疗组显著低于氨氯地平组和缬沙坦组(P<0.05),氨氯地平组和缬沙坦组比较差异无统计学意义(P>0.05)。联合治疗组治疗有效率为96.72%,氨氯地平组治疗有效率81.97%,缬沙坦组治疗有效率为80.33%,联合治疗组治疗效果显著高于氨氯地平组和缬沙坦组,差异有统计学意义(P<0.05)。治疗过程中联合治疗组不良反应发生率为9.8%,氨氯地平组不良反应发生率为6.6%,缬沙坦组为8.2%,3组比较差异无统计学意义(P>0.05)。结论缬沙坦氨氯地平片能够较为有效地控制老年高血压患者血压变异性,治疗效果较好。

Objective To investigate the effect of valsartan amlodipine on blood pressure variability in elderly patients with hypertension.Methods A total of 183 patients with hypertension admitted to Huidong People's Hospital from February 2018 to February 2019 were selected as the research subjects.According to the random number table method,they were assigned into an amlodipine group,a Valsartan group and a combination treatment group,with 61 cases in each group.After 6 months of treatment,blood pressure changes,blood pressure variations,clinical effects and adverse reactions during treatment were compared among the 3 groups.Results Before treatment,there were no statistically significant differences in blood pressure and blood pressure variation among the 3 groups(P>0.05).After treatment,daytime systolic/diastolic blood pressure,nighttime systolic/diastolic blood pressure,and morning peak systolic/diastolic blood pressure all decreased,and the blood pressure index of the combined treatment group was significantly lower than that of the amlodipine group and valsartan group(P<0.05),while there was no significant difference between amlodipine group and valsartan group(P>0.05).The 24 h mean systolic blood pressure and 24 h mean diastolic blood pressure in the 3 groups were significantly reduced(P<0.05),and the combination treatment group was significantly lower than the amlodipine group and valsartan group(P<0.05).There was no significant difference in mean diastolic blood pressure within 24 hours between the amlodipine group and the valsartan group(P>0.05).The variation coefficients of 24 h systolic blood pressure and 24 h diastolic blood pressure in the 3 groups were significantly reduced(P<0.05),and the combination treatment group was significantly lower than the amlodipine group and valsartan group(P<0.05).There was no significant difference between amlodipine group and valsartan group(P>0.05).The total effective rate of the combined treatment group was 96.72%,while the effective rate of the amlodipine group was 81.97%,and the effective rate of the valsartan group was 80.33%(P<0.05).The curative effects of combined treatment group was significantly higher than that of amlodipine group and valsartan group,the difference was statistically significant(P<0.05).During treatment,the incidence of adverse reactions in the combined treatment group was 9.8%,6.6%in the amlodipine group and 8.2%in the valsartan group,and there was no significant difference among the 3 groups(P>0.05).Conclusion Valsartan and amlodipine tablets can effectively control the blood pressure variability in elderly patients with hypertension,and the therapeutic effect is comparatively good.
