MUC5B、MUC6在骨肉瘤中的表达及意义

Expression and clinical significance of MUC5B and MUC6 in osteosarcoma

目的研究黏蛋白5B(MUC5B)、黏蛋白6(MUC6)在骨肉瘤中的表达及临床意义。方法选取2014年4月至2017年4月经手术切除的骨肉瘤标本67例,取骨纤维异常增殖73例和正常骨组织标本87例作为对照组,采用免疫组化法测定受试者骨肉瘤组织中MUC5B和MUC6表达水平,分析二者表达水平与骨肉瘤临床病理特征的关系,对所有术后患者进行随访,探究二者表达水平对骨肉瘤患者预后的影响。结果骨肉瘤组织中MUC5B和MUC6阳性表达率分别为79.10%(53/67)和71.46%(48/67),与对照组比较,MUC5B和MUC6阳性表达率显著上升,差异有统计学意义(P<0.05)。骨肉瘤组织中MUC5B和MUC6表达水平与患者的年龄、性别、肿瘤大小、Dahlin分类、Price分级无关,差异无统计学意义(P>0.05),与肿瘤的组织分化程度、有无肺转移有关,差异有统计学意义(P<0.05)。MUC5B高表达、MUC6高表达骨肉瘤患者术后生存率均显著低于MUC5B低表达、MUC6低表达患者,差异有统计学意义(P<0.05)。COX多因素分析表明,有肺转移、MUC5B高表达、MUC6高表达是影响骨肉瘤患者预后的独立危险因素,差异有统计学意义(P<0.05)。结论MUC5B和MUC6在骨肉瘤患者组织中表达水平显著升高,与骨肉瘤组织分化程度、有无肺转移及生存情况有关,可能作为评定骨肉瘤患者预后的临床指标。

Objective To investigate the expression and clinical significance of mucin 5B(MUC5B)and mucin 6(MUC6)in osteosarcoma.Methods A total of 67 osteosarcoma specimens that were removed surgically in our hospital from April 2014 to April 2017 were enrolled as observation group,at the same time,the other 73 cases of fibrous dysplasia and 87 cases of normal bone tissue specimens were taken as control group.The expression levels of MUC5B and MUC6 in osteosarcoma tissues were detected by immunohistochemistry,and the correlation between the expression levels and clinicopathological features of osteosarcoma was analyzed.And all the patients were followed up to observe the effects of the expression levels of MUC5B and MUC6 on the prognosis of patients with osteosarcoma.Results The expression positive rates of MUC5B and MUC6 in osteosarcoma tissues were 79.10%(53/67)and 71.46%(48/67),respectively,as compared with those in control group,the positive rates of MUC5B and MUC6 were increased significantly(P<0.05).The expression levels of MUC5B and MUC6 in osteosarcoma tissues were not correlated with patient’s age and sex,tumor size,Dahlin classification and Price classification(P>0.05),however,which were correlated with the degree of tissue differentiation and pulmonary metastasis(P<0.05).Moreover the survival rate of osteosarcoma patients with high expression of MUC5B and MUC6 was significantly lower than that of patients with low expression of MUC5B and MUC6(P<0.05).COX multivariate analysis showed that pulmonary metastasis,high expression of MUC5B and high expression of MUC6 were independent risk factors for the prognosis of patients with osteosarcoma(P<0.05).Conclusion The expression levels of MUC5B and MUC6 in osteosarcoma tissues are significantly increased,which is related to the degree of differentiation,lung metastasis and survival status of osteosarcoma,which may be used as clinical indexes to evaluate the prognosis of patients with osteosarcoma.
