PDCA循环联合静脉通路管理预防下肢静脉血栓的效果研究

Application effects of PDCA circulation management method in improving the management efficiency and management quality during the prevention of venous thrombosis of lower extremities

目的研究静脉通路管理预防下肢静脉血栓(DVT)期间使用PDCA循环管理方法降低静脉血栓发生率、提升护理管理质量。方法选取2018年1月至2020年1月收治的150例外科手术患者为研究对象,按随机数字表达分为对照组和观察组,每组75例。对照组予以常规静脉通路管理方法,观察组予以常规静脉通道管理联合PDCA循环管理方法,比较2组患者干预前后凝血功能变化情况、血流动力学改变、DVT发生率、DVT认知得分、护理满意度及护士管理质量。结果观察组干预前APTT、FIB、PT及D-D水平与对照组比较差异无统计学意义(P>0.05);干预后观察组APTT、PT水平比对照组更高,而FIB、D-D水平比对照组更低(P<0.05);干预前2组患者SCV、VRT及MVO比较差异无统计学意义(P>0.05),干预后观察组SCV、VRT及MVO、DVT认知得分、管理质量及护理满意度均比对照组更高(P<0.05)。结论在静脉通路管理中应用PDCA循环管理方法可有效预防患者发生DVT,值得临床应用。

Objective To investigate the application effects of PDCA circulation management method in improving the management efficiency and management quality during the prevention of lower limb venous thrombosis(DVT).Methods A total of 150 patients who underwent surgery in our hospital from January 2018 to January 2020 were enrolled in the study,who were divided into control group(n=75)and observation group(n=75)according to random digital table.The patients in control group were treated by conventional venous access management method,however,the patients in observation group were treated by conventional venous access management combined with PDCA circulation management method.The changes of coagulation function,hemodynamic changes,DVT incidence rate,DVT cognitive score,nursing satisfaction degree andnursing quality were observed and compared between the two groups.Results Before intervention there were no significant differences in the levels of APTT,FIB,PT and D-D between the two groups(P>0.05).After intervention the levels of APTT and PT in observation group were significantly higher than those in control group,however,the levels of FIB,DD were significantly lower than those in control group(P<0.05).Before intervention there were no significant differences in SCV,VRT and MVO between the two groups(P>0.05).After intervention,the SCV,VRT and MVO,DVT cognitive scores,management quality and nursing satisfaction degree in observation group were significantly higher than those in control group(P<0.05).Conclusion The application of PDCA circulation management method in the management of venous access can effectively prevent the incidence of DVT,therefor,which is worthy of clinical application.
