我院静脉药物配置中心1251例医嘱调配错误事件回溯性分析及干预策略探讨

Analysis and Intervention Strategies of 1251 Cases of Wrong Medical Order Dispatching in Venous Drug Disposition Center of Our Hospital

目的分析我院静脉药物配置中心医嘱调配错误事件并探究其干预策略。方法以回溯性分析方法将医嘱调配错误事件进行分类统计分析。结果 1 251例医嘱调配错误事件中包括溶剂不合理、给药频次不合理、浓度不当、超出加液量、低于加液量、配伍禁忌等,其中溶剂不合理、浓度不当、给药频次不合理、超出加液量为主要错误事件。结论静脉药物配置中心为药师提供良好的药学服务平台,应充分运用药学专业知识,并协助医师进行合理用药,以提高用药合理性及安全性。

Objective To analyze the misallocation of doctor’s orders and explore the intervention strategies of intravenous drug dispensing center in our hospital.Methods The error events of doctor’s orders were classified and analyzed by retrospective analysis.Results Among the 1251 cases of doctor’s order misallocation,the solvent was unreasonable,the frequency of administration was unreasonable,the concentration was improper,the amount of adding liquid was higher than that of adding liquid,and the compatibility was taboo.Conclusion The intravenous drug allocation center provides a good pharmaceutical service platform for pharmacists,and should make full use of pharmaceutical expertise and assist physicians rational use of drugs to improve the rationality and safety of drug use.
