安全腹腔镜胆囊切除术对胆结石患者疗效及安全性研究

Study on the Efficacy and Safety of Safe Laparoscopic Cholecystectomy in Patients with Cholelithiasis

目的:探讨安全腹腔镜胆囊切除术对胆结石患者疗效及安全性。方法:选取2017年5月-2019年12月泰州市第三人民医院普外科住院处胆结石患者120例,分为安全组及微创组,微创组采用传统腹腔镜腹胆囊切除术,安全组采用安全腹腔镜胆囊切除术,对比两组手术操作时间、术中失血量、术后首次排气时间以及术后住院时间,出血、感染、皮下气肿、胆管损伤等并发症。结果:两组手术时间差异无统计学意义(P>0.05),安全组术中失血量、术后首次排期时间及术后住院时间均少于微创组,差异有统计学意义(P<0.05),安全组术后仅有出血1例、无其他并发症,并发症发生率为1.67%,微创组术后出血3例、感染2例、胆管损伤2例,并发症发生率为11.6%,两组对比,差异有统计学意义(P<0.05)。结论:腹腔镜下胆囊切除术对胆结石患者疗效较好,安全性较高,值得推广应用。

Objective:To explore the efficacy and safety of safe laparoscopic cholecystectomy in patients with cholelithiasis.Methods:From May 2017 to December 2019,120 patients with cholelithiasis in the hospital were divided into safe group and minimally invasive group.Traditional laparoscopic cholecystectomy was used in the minimally invasive group and safe laparoscopic cholecystectomy was used in the safe group.The operation time,intraoperative blood loss,first postoperative exhaust time,the time of postoperative hospitalization,and bleeding,infection,subcutaneous emphysema,bile duct injury and other complications of the two groups were compared.Results:There was no significant difference in operation time between the two groups(P>0.05).The amount of blood loss during operation,the first postoperative exhaust time and the time of hospitalization in the safety group were less than those in the minimally invasive group,the difference was statistically significant(P<0.05).There was only one case of bleeding and no other complications in the safety group.The incidence of complications was 1.67%.There were 3 cases of bleeding,2 cases of infection and 2 cases of bile duct injury in the minimally invasive group,the incidence of the disease was 11.67%,the difference between the two groups was statistically significant(P<0.05).Conclusion:Laparoscopic cholecystectomy is effective and safe for patients with cholelithiasis,and is worthy of popularization and application.
