精准医学理念在肋骨骨折手术中的应用

Application of Precision Medicine Concept in Rib Fracture Surgery

目的探究精准医学理念在肋骨骨折手术中的应用效果。方法按照不同手术方式将71例肋骨骨折患者分为对照组(行传统手术,n=34)和观察组(行精准定位手术,n=37)。比较2组术中指标、手术前后疼痛VAS评分和肺功能指标(FEV1/%和MVV/%)以及术后和随访的并发症发生情况。结果观察组的手术时间、切口长度、术中出血量、引流管留置时间、自主下床活动时间和住院时间均明显少于对照组(均P<0.05);观察组术后2、5 d的VAS评分、FEV1/%和MVV/%均明显优于对照组(均P<0.05);观察组术后和术后1个月的并发症发生率明显低于对照组(35.39%比8.11%,χ^(2)=7.8587,P=0.0051;29.41%比5.41%,χ^(2)=7.2704,P=0.0070);2组术后2个月和3个月的并发症发生率比较,差异无统计学意义(均P>0.05)。结论肋骨骨折手术中,采用精准医学理念可有效定位患者骨折位置,改善手术时间、出血量、引流管留置时间等术中指标和肺功能指标,缓解术后疼痛,降低术后并发症发生率。

Objective To explore the efficacy of precision medicine concept in rib fracture surgery.Methods Seventy-one patients with rib fracture were assigned to receive either conventional surgery(control group,n=34)or precise positioning surgery(observation group,n=37).Intraoperative indicators,VAS pain score,lung function indices(FEV1/%and MVV/%)and incidence of complications were compared between the two groups.Results Compared with the control group,the operation time,incision length,intraoperative blood loss,drainage tube indwelling time,out-of-bed activity time and hospitalization time were reduced in the observation group(P<0.05).Furthermore,compared with the control group,the VAS score,FEV1/%and MVV/%were improved in the observation group at 2 and 5 days after surgery(P<0.05).In addition,the incidence of complications in the observation group was lower than that in the control group after surgery(35.39%vs 8.11%,χ^(2)=7.8587,P=0.0051)and at 1 month after surgery(29.41%vs 5.41%,χ^(2)=7.2704,P=0.0070).There was no significant difference in the incidence of complications between the two groups at 2 and 3 months after surgery(P>0.05).Conclusion In rib fracture surgery,precision medicine concept can effectively locate the fracture position,improve intraoperative indicators and pulmonary function indices,alleviate postoperative pain,and reduce the incidence of postoperative complications.
