多层螺旋CT血管成像及经胸超声心动图在复杂型主动脉缩窄及并发畸形中的应用

The application of MSCT angiography and transthoracic echocardiography in complex of coarctation of aorta and associated malformations

目的探讨多层螺旋CT血管成像(MSCTA)与经胸超声心动图(TTE)在复杂型主动脉缩窄(COA)及并发畸形中的应用。方法选取郑州大学附属儿童医院经手术确诊54例复杂型COA患儿资料,术前均经MSCTA和TTE检测。观察并比较两种检测方式的检出率及并发畸形检出情况。结果MSCTA与TTE对复杂型COA患儿检出率、缩窄段直径及长度检测结果比较,差异无统计学意义(P>0.05)。经手术证实54例患儿合并心内畸形63处,心外畸形58处。MSCTA对心内畸形的检出率低于TTE,对心外畸形的检出率高于TTE,差异有统计学意义(P<0.05)。结论MSCTA与TTE均可准确诊断复杂型COA,MSCTA对并发心外畸形的检出情况优于TTE,而TTE对心内畸形的检出情况较好;术前两者联合应用可为临床提供更为可靠的影像学信息。

Objective To investigate the application of multi-slice spiral CT angiography(MSCTA)and transthoracic echocardiography(TTE)in complex coarctation of aorta(COA)and associated malformations.Method The data of 54 children with complex COA diagnosed by surgery in Children’s Hospital Affiliated to Zhengzhou University were selected,and all of them were detected by MSCTA and TTE before surgery.To observe and compare the detection rate and concurrent deformity of the two methods.Result There was no statistically significant difference between MSCTA and TTE in the detection rate,diameter and length of narrow segment in children with complex COA.The operation confirmed that there were 63 intracardiac malformations and 58 extracardiac malformations in 54 cases.The detection rate of MSCTA for intracardiac malformations was lower than TTE,while the detection rate of extracardiac malformations was higher than TTE,with statistically significant differences(P<0.05).Conclusion Both MSCTA and TTE can accurately diagnose complex COA,and MSCTA is superior to TTE in detecting complicated extracardiac malformations,but inferior to TTE in detecting intracardiac malformations.Preoperative combined application of the two methods can provide more reliable imaging information for clinical practice.
